经鼻间歇正压通气和经鼻持续气道正压通气治疗早产儿呼吸暂停的临床效果比较 被引量:3

摘要 目的比较经鼻间歇正压通气(NIPPV)和经鼻持续气道正压通气(NCPAP)治疗早产儿呼吸暂停的临床效果。方法100例呼吸暂停早产儿随机分为两组各50例,间歇通气组予以NIPPV治疗,持续通气组予以NCPAP治疗,比较两组的呼吸功能恢复时间、住院时间、血气指标以及并发症发生率。结果间歇通气组的呼吸功能恢复时间及住院时间均短于持续通气组(P<0.05)。治疗后,两组的PaCO_(2)水平均低于治疗前,PaO_(2)水平均高于治疗前,且间歇通气组的PaCO_(2)水平低于持续通气组,PaO_(2)水平高于持续通气组(P<0.05)。间歇通气组的并发症发生率为8.00%,低于持续通气组的24.00%(P<0.05)。结论与NCPAP相比,NIPPV治疗早产儿呼吸暂停的效果更佳,可有效促进早产儿康复,改善早产儿血气指标,降低并发症发生率。 Objective To compare the clinical effects of nasal intermittent positive pressure ventilation(NIPPV)and nasal continuous positive airway pressure ventilation(NCPAP)in the treatment of premature apnea.Methods 100 cases of premature infants with apnea were randomly divided into two groups,with 50 cases in each group.The intermittent ventilation group was treated with NIPPV,and the continuous ventilation group was treated with NCPAP.The respiratory function recovery time,hospitalization time,blood gas indicators and incidence of complications were compared between the two groups.Results The respiratory function recovery time and hospitalization time of the intermittent ventilation group were shorter than those of the continuous ventilation group(P<0.05).After treatment,the PaCO_(2) levels of both groups were lower than those before treatment,and the PaO_(2) levels were higher than those before treatment;The PaCO_(2) level of the intermittent ventilation group was lower than that of the continuous ventilation group,and the PaO_(2) level was higher than that of the continuous ventilation group(P<0.05).The incidence of complications of the intermittent ventilation group was 8.00%,lower than 24.00%of the continuous ventilation group(P<0.05).Conclusions NIPPV has better effect than NCPAP in the treatment of premature apnea,which can effectively improve the recovery and blood gas indicators of premature infants,and reduce the incidence of complications.
