摘要
目的分析外周血miR-622表达与早期上皮性卵巢癌(EOC)患者经腹腔镜手术后复发的关系。方法选取2012年1月至2015年10月南方医科大学附属东莞医院收治的45例接受腹腔镜手术的早期EOC患者作为研究对象。依据术后5年随访是否复发分为复发组与未复发组,调查两组基线资料、检测实验室指标并比较,重点观察外周血中miR-622水平与早期EOC患者经腹腔镜手术后复发的关系。结果全部45例接受腹腔镜手术的早期EOC患者,经5年随访,复发26例,复发率为57.78%;初步比较两组基线资料及实验室指标后,经回归分析,外周血人附睾蛋白4过表达,miR-622低表达与早期EOC患者术后复发有关(OR>1,P<0.05);绘制受试者工作特征(ROC)曲线,结果显示,外周血人附睾蛋白4、miR-622水平预测早期EOC患者术后复发的曲线下面积(AUC)>0.80,且在外周血人附睾蛋白4、miR-622水平的cut-off值分别为41.415pmol/L、1.240时,预测价值较理想。结论早期EOC患者经腹腔镜手术后复发可能与术前外周血miR-622低表达有关。
Objective To analyze the relationship between the expression of miR-622 in peripheral blood and recurrence of patients with early epithelial ovarian cancer(EOC) after laparoscopic surgery. Methods 45 patients with early EOC who received laparoscopic surgery in Dongguan Hospital Affiliated to Southern Medical University from January 2012 to October 2015 were selected and divided into a recurrence group and a non recurrence group according to whether recurred during 5-year follow-up. Baseline data and laboratory indicators of the two groups were detected and compared, the relationship between the level of miR-622 in peripheral blood and the recurrence of patients with early EOC after laparoscopic surgery were mainly observed. Results Of the 45 patients with early EOC undergoing laparoscopic surgery, 26 patients had recurrence after 5 years of follow-up, and the recurrence rate was 57.78%. After preliminary comparison of baseline data and laboratory indicators between the two groups, regression analysis showed that overexpression of human epididymal protein 4 in peripheral blood and low expression of miR-622 were associated with postoperative recurrence in patients with early EOC(OR>1, P<0.05). The receiver operating characteristic(ROC) curve was drawn. The results showed that the area under the ROC curve(AUC) of peripheral blood human epididymal protein 4 and miR-622 in predicting postoperative recurrence in early EOC patients was greater than 0.80. When the cut-off value of peripheral blood epididymal protein 4 and miR-622 levels were 41.415 pmol/L and 1.240, respectively. The predictive value was ideal. Conclusions The recurrence of early EOC patients after laparoscopic surgery may be related to the low expression of miR-622 in preoperative peripheral blood.
