摘要
目的系统评价双排技术(double row, DR)与缝线桥技术(suture bridge, SB)治疗肩袖损伤的临床疗效。方法运用计算机检索PubMed、Embase、Cochrane Library、中国知网、Wanfang和中文VIP数据库,收集2010年1月1日~2021年1月10日国内外公开发表的采用DR和SB技术治疗肩袖损伤疗效对比的文献,筛选出符合纳入标准的文献并进行方法学质量评估,使用RevMan 5.3软件进行Meta分析。结果最终纳入13篇文献,包括1101名患者,其中DR组514名,SB组587名。结果表明,两种技术的术后肩关节功能评分(Constant评分)、美国肩肘外科协会评分(ASES评分)及术后前屈、外展、外旋角度差异均无显著统计学意义。SB组疼痛视觉模拟评分(VAS评分)、改良版美国加州大学肩关节评分(UCLA评分)均优于DR组(MD=0.21,95%CI:0.10~0.32,P=0.0002;MD=0.96,95%CI:0.21~1.71,P=0.01),且再撕裂率较低(MD=2.30,95%CI:1.45~3.64,P=0.0004)。结论 DR技术和SB技术均有助于肩袖损伤的治疗,SB技术的VAS和UCLA评分更好,且再撕裂率更低,值得推广应用。
Objective To evaluate the clinical efficacy of double row technique(doublerow, DR) and suture bridge technique(suturebridge, SB) in the treatment of rotator cuff injury.Methods The databases of PubMed, Embase, CochraneLibrary, China knowledge Network, Wanfang and Chinese VIP were searched by computer, and the literatures comparing the efficacy of DR and SB in the treatment of rotator cuff injury from January 1, 2010 to January 10, 2021 were collected. The literatures that met the inclusion criteria were selected and the methodological quality was evaluated. RevMan5.3 software was used for Meta analysis.Results 13 articles were included, including 1101 patients, including DR group(n = 514) and SB group(n = 587). The results showed that there was no significant difference in postoperative Constant score, ASES score, postoperative flexion, abduction and external rotation angle between the two techniques. The VAS score of SB group was better than that of DR group(MD=0.21,95%CI:0.10~0.32,P=0.0002), UCLA score was also better than that of DR group(MD=0.96,95%CI:0.21~1.71,P=0.01), and the retear rate was lower(MD=2.30,95%CI:1.45~3.64,P=0.0004). Conclusion Both DR technique and SB technique are helpful in the treatment of rotator cuff injury. SB technique has better VAS and UCLA scores and lower retear rate, so it is worth popularizing and applying.
