达格列净对早期糖尿病肾病患者肾功能及血清CRP、ICAM-1的影响 被引量:8

The effect of dapagliflozin on renal function and serum CRP and ICAM-1 in patients with early diabetic nephropathy

摘要 目的研究达格列净治疗2型糖尿病(T2DM)早期糖尿病肾病患者,血清CRP和ICAM-1的变化及对肾功能的影响。方法纳入受试者140例,其中健康对照组40例,2型糖尿病患者100例。2型糖尿病患者根据尿白蛋白/肌酐比值(UACR)分为2组:正常蛋白尿组50例(UACR<30mg/g),微量蛋白尿组50例(UACR 30~300mg/g),两组均使用达格列净10mg,1次/日,治疗12周。观察患者治疗前后体重指数(BMI)、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、收缩压(SBP)、舒张压(DBP)、肾小球滤过率(eGFR)、血肌酐(Scr)、尿素氮(BUN)、尿酸(UA)、血清C反应蛋白(CRP)和细胞间黏附因子(ICAM-1)的变化。结果治疗12周后,2型糖尿病患者BMI、HbA1c、SBP、FPG、eGFR、BUN均低于治疗前(P<0.05)。微量蛋白尿组治疗后UACR也较治疗前降低(P<0.05),正常蛋白尿组和微量蛋白尿组患者血清CRP、ICAM-1明显高于健康对照组,经治疗后均表现出显著下降(均P<0.05)。结论达格列净除了降低患者血糖、血脂、血压外也可显著减少尿蛋白,改善患者肾功能,其可能与降低患者血清CRP、ICAM-1有关。 Objective This study determined the changes in serum CRP and ICAM-1 in patients with type 2 diabetes(T2DM)and early diabetic nephropathy(DN)after treatment with dapagliflozin,and their effects on renal function.Methods A total of 140 subjects were enrolled,40 healthy controls and 100 T2DM patients were selected.The T2DM patients were divided into 2 groups according to the urine albumin/creatinine ratio(UACR):normal proteinuria group,50 cases(UACR<30mg/g);Microalbuminuria group,50 cases(UACR 30~300mg/g),normal proteinuria group and microalbuminuria group were treated with dapagliflozin 10mg,qd,for 12 weeks,observe the patient's body mass index(BMI)before and after treatment,Fasting blood glucose(FPG),glycosylated hemoglobin(HbA1c),systolic blood pressure(SBP),diastolic blood pressure(DBP),glomerular filtration rate(eGFR),blood creatinine(Scr),urea nitrogen(BUN),uric acid(UA),changes in serum C-reactive protein(CRP)and intercellular adhesion factor(ICAM-1).Results After 12 weeks of treatment,the BMI,HbA1c,SBP,FPG,eGFR,and BUN of the two groups were lower than before treatment(all P<0.05).UACR after treatment in the microalbuminuria group was also lower than before treatment(both P<0.05).Serum CRP and ICAM-1 in the normal proteinuria group and microalbuminuria group were significantly higher than those in the healthy control group,and both showed a significant decrease after treatment(All P<0.05).Conclusion In addition to reducing blood glucose,blood lipids,and blood pressure,dapagliflozin can also significantly reduce urine protein and improve renal function in patients,which may be related to the reduction of serum CRP and ICAM-1.
