摘要
目的分析动态心电图(dynamic electrocardiogram,DCG)与常规心电图(electrocardiogram,ECG)运用在冠心病(coronary heart disease,CHD)诊断中的效果及准确率。方法将2019年9月—2020年10月作为研究时间段,选取期间医院接诊且经冠状动脉造影(coronary angiography,CAG)确诊的50例CHD患者,进行ECG检查、DCG检查,对组间诊断准确率、ST段检出情况、ST段检出部位、心律失常类型展开分析。结果(1)DCG检查心肌缺血检出率(82.00%)、心律失常检出率(84.00%)均高于ECG检查(44.00%、52.00%),差异有统计学意义(P<0.05);(2)DCG检查ST段压低检出率(38.00%)、ST段抬高检出率(44.00%)均高于ECG检查(20.00%、24.00%),差异有统计学意义(P<0.05);(3)组间前间壁、前壁检出率差异无统计学意义(P>0.05);DCG检查下壁检出率(16.00%)、侧壁检出率(30.00%)均高于ECG检查(0、6.00%),差异有统计学意义(P<0.05);(4)DCG检查房室传导阻滞(38.00%)、右束支阻滞(38.00%)、左束支阻滞(40.00%)、房性早搏二/三联律(40.00%)、室性早搏二/三联律(50.00%)检出率均高于ECG检查(20.00%、20.00%、20.00%、20.00%、30.00%),差异有统计学意义(P<0.05)。结论对CHD患者进行DCG检查,可以提高心肌缺血以及心律失常检出率,并且还能评估心律失常分型。
Objective To analyze the effect and accuracy of dynamic electrocardiogram(DCG) and conventional electrocardiogram(ECG) in the diagnosis of coronary heart disease(CHD). Methods Taking September 2019 to October 2020 as the study time period, 50 CHD patients who were received in our hospital and diagnosed by coronary angiography(CAG)during the period were selected for ECG and DCG examinations. Analysis of the accuracy of inter-diagnosis, ST-segment detection, ST-segment detection location, and types of arrhythmia. Results(1) The detection rate of myocardial ischemia(82.00%) and arrhythmia(84.00%) in DCG examination were higher than those in ECG examination(44.00%, 52.00%),the difference was statistically significant(P < 0.05).(2) The detection rate of ST-segment depression in the DCG(38.00%) and ST-segment elevation(44.00%) were higher than those of ECG examinations(20.00%,24.00%), the difference was statistically significant(P < 0.05).(3) There was no significant difference in the detection rate of anterior septum and anterior wall between groups(P > 0.05). The detection rate of the inferior wall(16.00%) and the detection rate of the side wall(30.00%) of the DCG examination were higher than those of the ECG examination(0, 6.00%),the difference was statistically significant(P < 0.05).(4) DCG examination of atrioventricular block(38.00%), right bundle branch block(38.00%),left bundle branch block(40.00%), atrial premature beats two/triple rule(40.00%), The detection rate of the two/triple rule(50.00%) of ventricular premature beats was higher than that of ECG examination(20.00%,20.00%, 20.00%, 20.00%, 30.00%), the difference was statistically significant(P < 0.05). Conclusion DCG examination for CHD patients can improve the detection rate of myocardial ischemia and arrhythmia, and can also assess the arrhythmia classification.
