摘要
目的研究胃周“围刺”方法对肿瘤患者食欲的改善情况。方法选取2018年11月—2019年8月河北医科大学第四医院肿瘤内科病房和中医科门诊收治的伴有食欲减退的60例肿瘤患者,随机分为试验组30例、对照组30例。2组均给予西医基础支持治疗,部分患者行化疗。试验组在基础治疗的同时,加用针灸治疗,取穴巨阙、上脘、中脘、梁门(左)、足三里等,平补平泻手法。对照组口服醋酸甲地孕酮片,每日160 mg,1周为1个疗程(针刺5 d),观察1个疗程。观察2组患者食欲、体重、卡氏(KPS)评分、“痞满”症状积分、白蛋白的改善情况。观察严重不良事件发生情况。结果试验组在改善食欲,改善“痞满”症状,提高白蛋白水平方面与对照组相比优势明显(P<0.05),2组均未增加体重,差异无统计学意义(P>0.05)。治疗结束,2组患者KPS评分有效率比较,差异无统计学意义(P>0.05)。2组均未发现严重不良事件。结论胃周“围刺”可快速改善肿瘤患者食欲,提高患者生活质量,更易于为患者所接受。
Objective To investigate the therapeutic effects of acupuncture centered on the stomach in the treatment of appetiteloss caused by tumors.Methods Sixty patients with anorexia caused by tumors were assigned to the treatment group of 30 cases and the control group of 30 cases.Both groups were given basic modern medical support treatment,and some patients received chemotherapy.In the treatmentgroup,acupuncture treatment at such acupoints as RN 14,RN 13,RN 10,ST 21 and ST 36 was added to the basic treatment.The control group took megesterone acetate tablets orally,160mg per day.One week was one course of treatment,and we observed one course of treatment.Changes in appetite,body weight,Karnofsky performance score,abdominal fullness score and albumin were investigated in the two groups.Adverse events were also observed.Results As compared with the control group,the treatment group had obvious advantages in improving appetite,improving“abdominal fullness”symptoms,and increasing albumin levels(P<0.05).No weight gain was observed in both groups and the difference was not statistically significant(P>0.05).There was no significant difference in the KPS score between the two groups(P>0.05).No serious adverse events were found in either group.Conclusions Acupuncture centered on the stomach can quickly improve the appetite of cancer patients,improve the quality of life of patients,and be more acceptable to patients.
