摘要
目的了解某三级综合医院医院感染现患情况。方法采用横断面调查方法,对2018—2020年某三甲医院感染流行特征及病原菌送检等情况进行分析。结果 2018—2020年医院感染现患率分别为4.58%、4.52%和4.46%,差异无统计学意义。医院感染现患率较高的科室为ICU、儿科和外科;医院感染的感染部位主要为下呼吸道(51.63%)和泌尿道(27.31%)。3年检出病原菌主要以革兰阴性菌为主(67.89%);3次调查抗菌药物使用率分别为26.40%、25.99%、24.12%,呈下降趋势(χ^(2)=4.458,P<0.05);2018—2020年医院住院患者病原学送检率分别为28.40%、31.86%和31.28%,呈上升趋势(χ^(2)=3.979,P<0.05)。结论该医院部分科室仍需进一步加强医院感染预防和控制管理,减少经验用药,加强依据药敏结果指导用药的观念。
Objective To understand the current situation of nosocomial infection in a tertiary general hospital. Methods Cross-sectional survey method was used to understand the epidemiological characteristics of nosocomial infections and the inspection of pathogenic bacteria in 2018—2020. Results The prevalence rates of nosocomial infections from 2018 to 2020 were 4.58%, 4.52% and 4.46%, respectively. Departments with a high prevalence rate of nosocomial infections were ICU wards, pediatrics and surgery. The infection sites of nosocomial infections were mainly lower respiratory tract(51.63%) and urinary tract(27.31%). The main pathogens detected were Gram-negative bacteria(67.89%). The use rates of antibacterial drugs in the 3 surveys were 26.40%, 25.99% and 24.12% respectively, showing a downward trend(χ^(2)=4.458, P<0.05). The pathogenic examination rates of hospital inpatients from 2018 to 2020 were 28.40%, 31.86% and 31.28% respectively, showing an upward trend(χ^(2)=3.979, P<0.05).Conclusion The prevention and control management of nosocomial infection still need to be strengthened in parts of department, reducy empirical medication, and strengthening the concept of guiding drug based on drug sensitivity results are importrint.
