摘要
目的:观察滋阴补肾汤加减联合中药熏蒸治疗蒸发过强型中度干眼症的临床疗效。方法:选取蒸发过强型中度干眼症患者93例(186只眼),按照随机数字表法分为口服组、熏蒸组及联合组,每组31例(62只眼)。观察两组患者治疗前、治疗7 d后、治疗14 d后、治疗30 d后的主观症状、泪液分泌量、泪膜破裂时间(tear btreakup time,BUT)、角膜荧光素染色(cornea fluorescein staining,CFS)及生活质量评分。结果:联合组治疗7 d后、治疗14 d后、治疗30 d后主观症状评分高于口服组、熏蒸组,差异有统计学意义(P<0.05);治疗7 d后、治疗14 d后、治疗30 d后,联合组泪液分泌量、BUT高于口服组、熏蒸组,CFS低于口服组、熏蒸组,差异有统计学意义(P<0.05);联合组有效率83.87%,口服组有效率54.84%,熏蒸组有效率58.06%,联合组有效率高于口服组、熏蒸组,差异有统计学意义(P<0.05);联合组治疗30 d后生活质量评分高于口服组、熏蒸组,差异有统计学意义(P<0.05)。结论:滋阴补肾汤加减联合中药熏蒸治疗蒸发过强型中度干眼症,可有效增加泪液分泌量,延长BUT,提高患者生活质量。
Objective:To observe the clinical curative effect of Modified Yin-Nourishing and Kidney-Tonifying Decoction combined with fumigation with Chinese medicinals on moderate xerophthalmia of excessive evaporation type.Methods:A total of 93 patients(186 eyes)with moderate xerophthalmia of excessive evaporation type were randomly divided into the oral administration group,the fumigation group and the combined group,with 31 cases(62 eyes)in each group.Subjective symptoms,SchirmerⅠtest(SIT),tear break-up time(BUT),corneal fluorescein staining(CFS)and quality of life score were observed before treatment,7 days,14 days,and 30 days after treatment respectively.Results:The subjective symptom scores of the combined group after 7 days,14 days and 30 days of treatment were all significantly higher than those of the oral administration group and the fumigation group,and all the differences were statistically significant(P<0.05).After 7 days,14 days and 30 days of treatment,the amount of lacrimal secretion and BUT in the combined group were higher than those in the oral administration group and the fumigation group,while CFS was lower than those in the oral administration group and the fumigation group,and all the differences were statistically significant(P<0.05).The effective rate of the combined group was 83.87%,that of oral administration group was 54.84%,and that of fumigation group was 58.06%.The effective rate of the combined group was significantly higher than that of oral administration group and fumigation group,and all the differences were statistically significant(P<0.05).After 30 days of treatment,the quality of life score of the combined group was significantly higher than that of the oral group and that of the fumigation group(P<0.05).Conclusion:Modified Yin-Nourishing and Kidney-Tonifying Decoction combined with fumigation with Chinese medicinals in the treatment of moderate xerophthalmia of excessive evaporation type can effectively increase the amount of lacrimal secretion,prolong BUT and improve patients′quality of life.
