呼吸训练器对急性加重期慢性阻塞性肺疾病患者心肺功能及耐力的影响 被引量:13

摘要 目的呼吸训练器对急性加重期慢性阻塞性肺疾病(AECOPD)患者心肺功能及耐力的影响。方法选取南京医科大学附属淮安第一医院2017年9月~2020年9月98例AECOPD患者,随机数表法分为观察组与对照组,各49例。对照组在患者病情相对稳定后行缩唇式腹式呼吸训练,观察组在对照组基础上采用呼吸训练器进行呼吸锻炼,两组均持续锻炼2周。记录两组入住重症监护室(ICU)时间、平均住院时间,测定两组干预前、干预结束1周后心肺功能指标[N端脑利钠肽前体(NT-proBNP)、左心射血分数(LVEF)、最大呼气峰流速值(PEF)、用力呼气容积/用力肺活量(FEV1/FVC)],观察两组干预结束1周后6 min步行距离(6MWD)、改良版英国医学研究学会呼吸困难量表(MMRC)及慢性阻塞性肺疾病(COPD)评估测试评分(CAT),测定两组干预前和动脉血气指标[pH值、动脉氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、氧合指数(OI)]。结果观察组入住ICU时间、平均住院时间均显著低于对照组,差异有统计学意义(P<0.05)。干预后观察组NT-proBNP显著低于本组干预前及对照组干预后,LVEF、PEF、FEV1/FVC显著高于本组干预前及对照组干预后,差异有统计学意义(P<0.05)。干预后观察组6MWD显著高于对照组,MRC、CAT评分显著低于对照组,差异有统计学意义(P<0.05)。干预后观察组pH、PaO_(2)、OI显著高于本组干预前及对照组干预后,PaCO_(2)显著低于本组干预前及对照组干预后,差异有统计学意义(P<0.05)。结论呼吸训练器可有效改善AECOPD患者心肺功能,提高患者耐力。 Objective To investigate the effect of respiratory training device on cardiopulmonary function and endurance in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods 98 patients with AECOPD in our hospital were randomly divided into observation group and control group,49 cases in each group.The control group accepted routine pursed lips ventral breathing training after the patient's condition was relatively stable,and the patients in the observation group were given abdominal respiratory function exercise on the basis of the control group by using respiratory trainer,and both groups continued to exercise for 2 weeks.ICU stay and average length of stay were recorded,The indexes of cardiopulmonary function[N-terminal pro brain natriuretic peptide(NT-proBNP),left ventricular ejection fraction(LVEF),peak expiratory flow(PEF),forced expiratory volume/forced vital capacity(FEV1/FVC)]were measured before and 1 week after the intervention,6MWD,MMRC and cat were observed after intervention,The arterial blood gas indexes[pH,PaO_(2),PaCO_(2),OI]were measured before intervention.Results The ICU stay and average hospital stay of the observation group were significantly lower than those of the control group(P<0.05).After the intervention,NT-proBNP in the observation group was significantly lower than that before the intervention and after the intervention in the control group,while LVEF,PEF and FEV1/FVC in the observation group were significantly higher than those before the intervention and after the intervention in the control group(P<0.05).After the intervention,the 6MWD of the observation group was significantly higher than that of the control group,and the MRC and cat scores of the observation group were significantly lower than those of the control group(P<0.05).After the intervention,pH,PaO_(2)and oi of the observation group were significantly higher than those before the intervention and after the intervention of the control group,while PaCO_(2)was significantly lower than those before the intervention and after the intervention of the control group(P<0.05).Conclusion The respiratory trainer can effectively improve the cardiopulmonary function and endurance of AECOPD patients.
