输血对极低出生体重早产儿生命体征及喂养的影响 被引量:1

摘要 目的探讨输血对极低出生体重早产儿生命体征及喂养的影响。方法选取2017年5月至2019年7月医院NICU收治的符合纳入标准的408例极低出生体重早产儿,回顾性分析其中输血患儿输血前后的临床资料;并根据喂养不耐受标准将输血患儿分为喂养耐受组与喂养不耐受组,比较两组的临床特征。结果最终接受输血患儿303例;患儿输血后24 h的血红蛋白、血细胞比容、血氧饱和度、舒张压均高于输血前,呼吸、心率、呼吸暂停频率均低于输血前,差异有统计学意义(P<0.05);为了评估输血是否对喂养产生影响,特剔除输血前50例喂养不耐受以减少偏倚,于输血24 h后将剩余253例患儿根据喂养不耐受标准分为喂养耐受组(210例)与喂养不耐受组(43例),结果显示,喂养不耐受组出生体重及输血时体重均低于喂养耐受组,输血次数多于喂养耐受组,差异有统计学意义(P<0.05);且研究中未发现输血后出现坏死性小肠结肠炎的病例。结论极低出生体重早产儿经输血治疗后呼吸、心率下降,间歇性低氧发作减少;且体重越低、输血次数越多,输血后发生喂养不耐受的风险越高。 Objective The effects of blood transfusion on vital signs and feeding of very low birth weight premature infants were explored. Methods A total of 408 very low birth weight infants who could meet the enrolled standards admitted to the NICU of the hospital from May 2017 to July 2019 were selected. The clinical data before and after blood transfusion of the infants treated by blood transfusion were retrospectively analyzed. According to the feeding intolerance standard, the infants who have received blood transfusion were divided into a feeding tolerance group and a feeding intolerance group, and then the clinical features of the two groups were compared. Results A total of 303 infants received blood transfusion eventually. 24 hours after blood transfusion, the levels of hemoglobin, hematocrit, blood oxygen saturation and diastolic blood pressure were all higher than before blood transfusion, the respiration rate, the heart rate and the apnea attacks rate were all lower than before blood transfusion, with statistical y significant differences(P<0.05);In order to evaluate weather blood transfusion have effects on feeding or not, 50 feeding intolerance infants were eliminated especially to reduce derivation, 24 hours after blood transfusion, the remained 253 infants were divided to a feeding tolerance group(210 cases) and a feeding intolerance group(43 cases) according to the feeding intolerance standard, the results showed that the weight of the feeding intolerance group at birth and after blood transfusion were both lower than those of the feeding tolerance group, the transfusion times were more than that of the feeding tolerance group, with statistically significant differences(P<0.05);No cases of necrotizing enterocolitis were found after transfusion in this study. Conclusions After blood transfusion, the respiration and heart rate of the very low birth weight infants decreased, and meanwhile the intermittent hypoxic episodes decreased. The lower the body weight and the more the transfusions are implemented, the higher the risks of feeding intolerance after transfusions are.
