摘要
目的:了解社会经济地位对四川省65~74岁老年人牙周健康状况的影响,为四川省老年人口腔卫生保健工作提供信息支持。方法:采用多阶段分层随机抽样方法,抽取四川省65~74岁老年人共744名,男女各半,城乡各半。采用第四次全国口腔健康流行病学调查制定的方法和标准进行调查,对户口类型、学历水平、家庭总收入等情况进行问卷调查。结果:户口类型对老年人的牙龈出血状况(P=0.001)和牙周附着丧失(P=0.040)均有影响:相比于农业户口的老年人而言,非农业户口的老年人更少出现广泛性牙龈出血(AOR=0.479,95%CI:0.314,0.732,P=0.001)。非参数检验发现学历水平影响老年人牙龈出血状况(P=0.011),家庭收入对老年人牙周状况的影响无统计学差异。结论:四川省65~74岁老年人的牙周健康状况普遍较差,即使在学历和家庭收入水平更高的老年群体中也是一样。应加强对老年群体的口腔健康教育与服务,尤其是对农业户口老年人的口腔卫生保健服务。
Objective:To investigate the association between socio-economic status and periodontal health status in the elderly aged 65-74 years in Sichuan province,and provide information support for oral health care of the elderly peoplein Sichuan province.Methods:A total of 744 elderly people aged 65-74 years old in Sichuan province were selected by multi-stage stratified random sampling method.The methods and standards developed by the fourth National Oral health Epidemiological survey were adopted.A questionnaire survey was conducted on type of household,education and household income,ect.Results:Different types of household had effects on gingival bleeding(P=0.001)and loss of periodontal attachment(P=0.040)in the elderly people.The elderly from non-agricultural household were less likely to have gingival bleeding than those from agricultural household(AOR=0.479;95%CI:0.3140.732;P=0.001).Education level has an impact on gingival bleeding in the elderly people from non-parametric test(P=0.011).In addition,household income has no statistically significant impact on periodontal status in the elderly people.Conclusion:The periodontal health status of the elderly people aged 65-74 years in Sichuan province is relatively poor,even among the elderly people with higher educational background and household income.Oral health education and services should be strengthened for the elderly people,especially the oral health care services for the elderly people from agricultural families.
