摘要
目的分析增生性糖尿病性视网膜病变(PDR)患者玻璃体切除术后新生血管性青光眼(NVG)发生的相关因素。方法回顾性分析2018年5月至2020年12月郑州市第二人民医院收治PDR者行玻璃体切除手术208例(208只眼)的临床资料。根据术后情况分为NVG组和非NVG组, 其中NVG组21例(21只眼), 非NVG组187例(187只眼)。术后随访(10.3±1.6)个月, 分析术后发生NVG的相关因素。结果随访末期, 出现NVG者21只眼(10.1%)。与玻切术后NVG呈正相关的因素有:术前患者血红蛋白(OR=1.473, 95%CI:1.191~1.822, P=0.002)、同型半胱氨酸水平(OR=1.064, 95%CI:1.005~1.857, P=0.038), 术后玻璃体积血(OR=37.832, 95%CI:1.558~98.549, P=0.026)、视网膜脱离(OR=3.322, 95%CI:0.338~32.651, P=0.001)。结论术前高血红蛋白、同型半胱氨酸水平及术后玻璃体积血、视网膜脱离是糖尿病性视网膜病变患者玻璃体切除术后发生新生血管性青光眼的危险因素。
Objective To analyse the related factors of neovascular glaucoma(NVG)after vitrectomy in proliferative diabetic retinopathy(PDR)patients.Methods The clinical data of 208 eyes of 208 cases with PDR undenvent vitrectomy from May 2018 to Der.2020 in Zhengzhou Municipal Second People's Hospital were analyzed retrospectively.All cases were divided into two groups based on their postoperative conditions.There were 21 eyes of 21 rases in the NVG group,and 187 eyes of 187 cases in the non-NVG group.The average follow-up time after vitrectomy was(10.3±1.6)monlhs.The related factors of NVG after ojieration were analyzed.Results NVG occurred in 21 eyes of 21 cases,and the incidence was 10.1%at the end of the follow-up.There were positive correlations with the occurrence of NVG after vitrectomy:high preoperative hemoglobin(OR=1.413,95%CI:1.191-1.822,P=0.002),homocysteine(OR=1.064,95%CI:1.005-1.857,P=0.038),the postoperative vitreous hemorrhage(OR=31.832,95%CI:1.558-98.549,P=0.026)t and retinal detachment(OR=3.322,95%CI:0.338-32.651,P=0.001).Conclusion Preoperative high hemoglobin and homocysteine levels,postoperative vitreous hemorrhage and retinal detachment are risk factors for neovascular glaucoma after vitrectomy in patients with PDR.
