尿常规联合有形成分分析诊断泌尿系感染的价值分析

Value analysis of the urine routine examination combined with the analysis of visible urine components in diagnosing urinary tract infection

摘要 目的评价尿常规联合有形成分分析诊断泌尿系感染的价值。方法80例泌尿系感染患者,均进行尿常规单独检测(单一检测),尿常规联合有形成分分析检测(联合检测)。比较联合检测及单一检测的诊断结果;以尿培养结果为金标准,分析阳性病原菌检测情况。结果联合检测的阳性率80.00%明显高于单一检测的50.00%,差异具有统计学意义(P<0.05)。联合检测中,亚硝酸盐(NIT)阳性48例(60.00%)、尿白细胞酯酶(LEU)阳性64例(80.00%)、尿白细胞(WBC)阳性72例(90.00%)。尿培养结果显示:①革兰阴性菌48例(60.00%),其中大肠埃希菌26例(54.17%),克雷伯菌属6例(12.50%),肠杆菌属4例(8.33%),变形菌属4例(8.33%),阴道加德纳菌4例(8.33%),动杆菌属1例(2.08%),铜绿假单胞菌1例(2.08%),弗氏柠檬酸杆菌1例(2.08%),其他1例(2.08%);②革兰阳性菌28例(35.00%),其中链球菌属10例(35.71%),葡萄球菌属6例(21.43%),肠球菌属6例(21.43%),格氏乳球菌1例(3.57%),棒杆菌属1例(3.57%),乳杆菌属1例(3.57%),短稳杆菌1例(3.57%),放线菌属1例(3.57%),纽氏放线菌纽氏亚种1例(3.57%);③真菌4例(5.00%),其中念珠菌属3例(75.00%),酵母菌1例(25.00%)。结论与单一检测比较,联合尿常规、尿液有形成分分析检测有助于了解泌尿系感染患者的病变信息,为临床工作提供参考信息。 Objective To evaluate the value of urine routine examination combined with the analysis of visible urine components in the diagnosis of urinary tract infection.Methods 80 patients with urinary tract infection were subjected to the sole urine routine examination(single test)and the urine routine examination combined with the analysis of visible urine components(combined test).The diagnostic results of combined test and single test were compared.Results of the urine culture was used as the gold standard,and the detection of positive pathogens was analyzed.Results The positive rate of combined detection was 80.00%,which was significantly higher than 50.00%of single detection,and the difference was statistically significant(P<0.05).In the combined detection,48 cases(60.00%)had positive nitrite(NIT),64 cases(80.00%)had positive leukocyte esterase in urine(LEU),and 72 cases(90.00%)had positive white blood cell in urine(WBC).The results of urine culture showed that①Gram-negative bacteria were found in 48 cases(60.00%).Among them,there were 26 cases(54.17%)of Escherichia coli,6 cases(12.50%)of Klebsiella,4 cases(8.33%)of Enterobacter,4 cases(8.33%)of Proteus,4 cases(8.33%)of Gardnerella vaginalis,1 case(2.08%)of Acinetobacter,1 case(2.08%)of Pseudomonas aeruginosa,1 case(2.08%)of Citrobacter freundii and 1 case(2.08%)of others.②Gram-positive bacteria were found in 28 cases(35.00%).Among them,there were 10 cases(35.71%)of Streptococcus,6 cases(21.43%)of Staphylococcus,6 cases(21.43%)of Enterococcus,1 case(3.57%)of Lactococcus griffithi,1 case(3.57%)of Corynebacterium,1 case(3.57%)of Lactobacillus,1 case(3.57%)of Brevibacterium,1 case(3.57%)of actinomycetes and 1 case(3.57%)of actinomycetes subspecies neoformans.③Fungi were found in 4 cases(5.00%).Among them,there were 3 cases(75.00%)of Candida and 1 case(25.00%)of yeast.Conclusion Compared with the single test,the urine routine examination combined with the analysis of visible urine components is helpful to collect the pathological information of patients with urinary tract infection,so as to provide information for clinical practice.
