摘要
目的探讨癫痫患者血清锌α2糖蛋白(zinc alpha-2 glycoprotein,ZAG)水平与认知功能、血清炎症细胞因子的关系。方法选取我院2019年7月至2021年3月收治的癫痫患者104例进行研究,将其作为癫痫组,纳入同期于我院体检的健康志愿者83例作为对照组。比较两组血清ZAG、白介素-1β(interleukin-1β,IL-1β)、白介素-2(interleukin-2,IL-2)、白介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平,利用蒙特利尔认知评估量表(Montreal cognitive assessment,MoCA)评估两组认知功能。观察血清ZAG水平与癫痫患者临床病理特征的关系。经Pearson线性相关分析癫痫患者血清ZAG水平与MoCA评分、炎症因子水平的相关性。绘制受试者工作特征曲线(receiver operating characteristic curve,ROC)分析血清ZAG评估癫痫的曲线下面积(area under curve,AUC)。结果癫痫组血清IL-1β、IL-2、IL-6、TNF-α水平较对照组升高,而血清ZAG水平及MoCA评分较对照组降低(P<0.05)。癫痫患者低表达组病程>5年、全面性发作、癫痫样波、重度癫痫占比高于高表达组(P<0.05)。Pearson线性相关分析提示癫痫患者血清ZAG水平与IL-1β、IL-2、IL-6、TNF-α呈负相关,与MoCA评分呈正相关(P<0.05)。血清ZAG水平单独评估癫痫的AUC为0.785(95%CI:0.712~0.857,P<0.05)。血清ZAG联合MoCA评分及炎症因子评估癫痫的AUC为0.914(95%CI:0.864~0.964,P<0.05)。结论癫痫患者血清ZAG水平明显下调,且其与炎症因子呈负相关,与认知障碍程度则呈正相关,对癫痫评估有一定价值,临床有望将其作为诊断癫痫的重要依据。
Objective To explore the relationship between serum zincα2 glycoprotein(ZAG)level and cognitive function and serum inflammatory cytokines in patients with epilepsy.Methods A total of 104 epilepsy patients admitted to our hospital from July,2019 to March,2021 were selected for the study.These patients were enrolled as the epilepsy group,while 83 healthy volunteers who received physical examination in our hospital during the same period were included as the control group.Serum levels of ZAG,interleukin-1β(IL-1β),interleukin-2(IL-2),interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α)were compared between the two groups and patients were also assessed by the Montreal cognitive assessment(MoCA)for two sets of cognitive functions.We evaluated the relationship between serum ZAG level and clinicopathological characteristics of patients with epilepsy.The correlation between serum ZAG level and MoCA score and inflammatory factor levels in epilepsy patients was analyzed by Pearson linear correlation.A receiver operating characteristic curve(ROC)was drawn to calculate the area under the curve(AUC)of serum ZAG to assess epilepsy.Results Levels of serum IL-1β,IL-2,IL-6,and TNF-αin the epilepsy group were higher than those in the control group,while serum ZAG level and MoCA score were lower than those in the control group(P<0.05).The proportion of epilepsy patients with low expression group>5 years,generalized seizures,epileptiform waves,and severe epilepsy were higher than those in the high expression group(P<0.05).Pearson linear correlation analysis showed that serum ZAG levels in patients with epilepsy were negatively correlated with IL-1β,IL-2,IL-6,and TNF-α,while positively correlated with MoCA scores(P<0.05).AUC of epilepsy assessed by serum ZAG level alone was 0.785(95%CI:0.712-0.857,P<0.05).AUC of epilepsy assessed by serum ZAG combined with MOCA score and inflammatory factors was 0.914(95%CI:0.864-0.964,P<0.05).Conclusion Serum ZAG level in patients with epilepsy is significantly down-regulated,and it is negatively correlated with inflammatory factors,and positively correlated with the degree of cognitive impairment.It has a certain value in the evaluation of epilepsy,which can be used as an important clinical reference for the diagnosis of epilepsy.
