摘要
目的探讨老年脑梗死患者颅内外动脉支架植入术前后血糖与脑血流动力学的相关性。方法收集2018年7月至2021年7月安徽医科大学附属六安医院,安徽省六安市人民收治的126例颅内外动脉支架植入手术治疗的老年脑梗死患者临床资料。比较126例患者术前1 d和术后3 d的空腹血糖水平、脑血流动力学指标[动脉舒张期峰值(Vd)、收缩期血流速度峰值(Vs)、平均血流速度(Vm)、搏动指数(PI)];分析空腹血糖水平与脑血流动力学指标的相关性。分析脑血流动力学对空腹血糖水平的影响。结果术后3 d,126例老年脑梗死患者空腹血糖水平和PI高于术前1 d,Vd、Vs和Vm低于术前1 d,差异有统计学意义(P<0.01)。126例患者空腹血糖水平与Vd、Vs和Vm呈负相关,与PI呈正相关(P<0.05)。Vd、Vs、Vm是空腹血糖水平的保护因素,PI是空腹血糖水平的危险因素(R^(2)=0.831,F=41.358,P<0.05)。结论老年脑梗死患者接受颅内外动脉支架植入手术后3 d,血糖水平上升,脑血流动力学也发生变化,两种指标间具有密切的相关性,评估两种指标有助于对患者的风险评估及预后预测。
Objective To investigate the correlation of blood glucose with cerebral hemodynamics in elderly patients with cerebral infarction before and after intracranial and extracranial artery stent implantation.Method The clinical data of 126 elderly patients with cerebral infarction treated by intracranial and extracranial artery stent implantation in Lu'an Hospital of Anhui Medical University,Lu'an People's Hospital of Anhui Province from July 2018 to July 2021 were collected.The levels of fasting blood glucose and cerebral hemodynamics[peak diastolic velocity(Vd),peak systolic velocity(Vs),mean blood velocity(Vm)and pulsatility index(PI)]were 1 day before operation and 3 days after operation.The correlation of fasting blood glucose with cerebral hemodynamics was analyzed,and the influencing factors of cerebral hemodynamics on fasting blood glucose were analyzed.Result Three days after operation,fasting blood glucose and PI in 126 elderly patients with cerebral infarction were higher than those 1 day before operation,and Vd,Vs and Vm were lower than those 1 day before operation(P<0.01).The fasting blood glucose of 126 patients was negatively correlated with Vd,Vs and Vm,and positively correlated with PI(P<0.05).Vd,Vs and Vm were protective factors of fasting blood glucose level,and PI was the risk factor of fasting blood glucose level(R^(2)=0.831,F=41.358,P<0.05).Conclusion Three days after intracranial and extracranial artery stent implantation,the blood glucose increases and the cerebral hemodynamics also changes.There is a close correlation between the two indexes.Evaluating the two indexes is helpful to the risk assessment and prognosis of patients.
