摘要
目的探讨导乐陪伴分娩在初产妇温柔分娩护理模式中的应用。方法方便选取2019年1月—2021年1月于该院分娩的初产妇134例,随机分为两组。对照组应用常规分娩护理,研究组应用温柔分娩理念及其指导下助产士全程导乐陪伴分娩,观察两组产妇分娩前后的心理状况、剖宫产率、产程情况以及产后出血情况。结果研究组第一产程(424.74±142.27)min、第二产程(54.17±21.04)min、第三产程时间(7.64±2.81)min均短于对照组(507.63±174.41)、(63.26±19.53)、(8.75±3.18)min,差异有统计学意义(P<0.05);研究组产后2 h内产后出血发生率(2.99%)、4 h内产后出血发生率(1.49%)、剖宫产率(7.46%)均比对照组低(16.42%、13.43%、23.88%),差异有统计学意义(χ^(2)=5.452、5.295、5.647,P<0.05);研究组SDS(37.8±3.10)分、SAS评分(36.7±3.10)分均低于对照组(45.4±3.30)分、(44.7±3.70)分,差异有统计学意义(t=13.740、13.566,P<0.05);研究组满意度(95.52%)比对照组(82.09%)高,差异有统计学意义(χ^(2)=6.081,P<0.05);研究组与对照组产后感染、产后出血、尿潴留、胎儿窘迫、妊高征发生率均差异无统计意义(P>0.05)。结论导乐陪伴分娩在初产妇温柔分娩护理模式中的应用效果较好,可以有效降低其焦虑和抑郁情况,有利于产程的缩短,与此同时其还能有效降低产后出血的发生率以及并发症的发生率,提高满意度。
Objective To explore the application of Doula accompany delivery in the gentle delivery nursing model of primiparas.Methods A total of 134 primiparas who gave birth in this hospital from January 2019 to January 2021 were selected and randomly divided into two groups.The control group used routine delivery care,and the study group used the concept of gentle childbirth and guided the midwife to accompany the delivery throughout the delivery.The psychological status,cesarean section rate,delivery process and postpartum hemorrhage of the two groups were observed before and after delivery.Results The first stage of labor(424.74±142.27)min,the second stage of labor(54.17±21.04)min,and the time of the third stage of labor(7.64±2.81)min in the study group were shorter than those in the control group(507.63±174.41)min,(63.26±19.53)min,(8.75±3.18)min,the difference was statistically significant(P<0.05);the incidence of postpartum hemorrhage within 2 h of the study group(2.99%)and the incidence of postpartum hemorrhage within 4 h(1.49%)and the cesarean section rate(7.46%)were lower than those of the control group(16.42%,13.43%,23.88%),and the difference was statistically significant(χ^(2)=5.452,5.295,5.647,P<0.05);the study group SDS(37.8±3.10)points and SAS score(36.7±3.10)points were lower than the control group(45.4±3.30)points and(44.7±3.70)points,the difference was statistically significant(t=13.740,13.566,P<0.05);The satisfaction of the study group(95.52%)was higher than that of the control group(82.09%),and the difference was statistically significant(χ^(2)=6.081,P<0.05);the study group and the control group had postpartum infection,postpartum hemorrhage,urinary retention,fetal distress,pregnancy,there was no statistically significant difference in the incidence of high symptoms(P>0.05).Conclusion Doula escort delivery is effective in the gentle delivery nursing mode of primiparas.It can effectively reduce their anxiety and depression,and is beneficial to shorten the labor process.At the same time,it can effectively reduce the incidence of postpartum hemorrhage and complication rate and increase satisfaction.
