摘要
目的分析切开复位哈勃钉内固定植骨联合桡骨茎突筋膜骨瓣治疗陈旧性腕舟状骨骨折的效果及对腕关节功能的影响。方法选取2016年1月—2020年1月本院治疗的陈旧性腕舟状骨骨折118例,按照治疗方式的不同均分为研究组(切开复位哈勃钉内固定植骨联合桡骨茎突筋膜骨瓣治疗)和对照组(单纯哈勃钉内固定联合松质骨植骨)。比较2组临床疗效,围术期指标(手术时间、骨折愈合时间、术中出血量、住院时间),Mayo腕关节功能评分及术后并发症发生情况。结果研究组总有效率显著高于对照组(P<0.01)。研究组手术时间长于对照组,骨折愈合时间及住院时间短于对照组,术中出血量少于对照组(P<0.01)。研究组治疗后Mayo腕关节功能评分优于对照组(P<0.05,P<0.01)。研究组术后并发症总发生率明显低于对照组(P<0.05)。结论切开复位哈勃钉内固定植骨联合桡骨茎突筋膜骨瓣治疗陈旧性腕舟状骨骨折效果显著,可加速患者骨折愈合,利于术后腕关节功能恢复,且降低了术后并发症发生率。
Objective To analyze clinical efficacy of open reduction,Hubble nail internal fixation and bone grafting combined with radial styloid fascia bone flap in treatment of patients with old fracture of scaphoid bone of wrist and its effect on wrist function.Methods A total of 118 patients with old fracture of scaphoid bone of wrist admitted between January 2016 and January 2020 were divided into research group(open reduction,Hubble nail internal fixation and bone grafting combined with radial styloid fascia bone flap)and control group(simple Hubble nail internal fixation combined with cancellous bone grafting).Clinical efficacy,perioperative indexes(operative duration,fracture healing time,intraoperative volume of bleeding,length of hospital stay),Mayo wrist function score and incidence rate of postoperative complications were compared between two groups.Results The total effective rate in research group was significantly higher than that in control group(P<0.01).Compared with those in control group,operative duration was significantly longer,and fracture healing time and length of hospital stay were significantly shorter,and intraoperative volume of bleeding was less in research group(P<0.01).After treatment,Mayo wrist function score in research group was better than that in control group(P<0.05,P<0.01).The total incidence rate of postoperative complications in research group was significantly lower than that in control group(P<0.05).Conclusion Open reduction,Hubble nail internal fixation and bone grafting combined with radial styloid fascia bone flap is effective in treatment of patients with old carpal scaphoid fractures of wrist.It may accelerate fracture healing,facilitate recovery of wrist function and reduce incidence rate of postoperative complications.
