早产儿喂养方式对其Ghrelin、Leptin、血脂水平及生长发育的影响 被引量:4

EFFECTS OF FEEDING METHODS ON GHRELIN,LEPTIN,BLOOD LIPID LEVEL AND GROWTH AND DEVELOPMENT OF PREMATURE INFANTS

摘要 目的分析讨论不同喂养方式对早产儿饥饿素(Ghrelin)、瘦素(Leptin)、血脂水平及生长发育的影响。方法选取2018年1—12月在南充市中心医院接受治疗的113例早产儿(非混合喂养)为主要研究对象,按照随机数字表法将早产儿分为A组(n=56,采用母乳喂养),B组(n=57,采用配方奶喂养),观察二组早产儿出院后3个月、6个月时饥饿素、瘦素、血脂水平变化及生长发育情况。结果出院后3个月,A组的瘦素、饥饿素水平均明显高于B组,差异有统计学意义(P<0.05);出院后6个月,B组的瘦素明显高于A组,差异有统计学意义(P<0.05);二组出院后体重、头围及身长的增长幅度呈出院后6个月>出院后3个月>出院时的趋势,差异均有统计学意义(均P<0.05);A组出院后3个月的各项生长发育指标高于B组,差异均有统计学意义(均P<0.05);且B组出院后6个月的体重及身长增长均显著高于A组,差异均有统计学意义(均P<0.01)。A组正常增长,B组持续稳步增长至超重。结论出院后3个月内母乳喂养更能促进早产儿的生长发育,但与出院后6个月的母乳喂养相比,早产儿配方奶能促进早产儿的生长发育。 Objective To analyze and discuss the effects of different feeding methods on Ghrelin, Leptin, blood lipid levels and growth and development of premature infants.Methods One hundred and thirteen premature infants(non-mixed feeding) treated in our hospital from January 2018 to December were selected as main study subjects.The preterm infants were divided into A group(n=56,breastfeeding) according to the random number table method.Group B(n=57,fed with formula);The changes of blood lipid level and growth and development of Ghrelin, Leptin in 3 months and 6 months old preterm infants in two groups were observed.Results The level of Ghrelin, Leptin in group A was significantly higher than that of group B(P<0.05).At 6 months after discharge, the Leptin in group B was significantly higher than that in group A(P< 0.05).The increase of body weight, head circumference and length of the two groups after discharge was 6 months after discharge> 3 months after discharge> the trend of discharge, the difference was statistically significant(P<0.01);The growth and development indexes in group A were significantly higher than those in group B at 3 months after discharge(P<0.05);The body weight and body length in group B were significantly higher thanthose in group A at 6 months after discharge(P<0.01).Group A grew normally, while group B continued to grow steadily to overweight.Conclusion Breast feeding could promote the growth and development of premature infants within 3 months after discharge, but compared with breast feeding at 6 months after discharge, the formula of premature infants could promote the growth and development of preterm infants.
