摘要
目的探讨规范营养支持对有营养风险的胃肠道肿瘤患者围术期结局的影响。方法回顾性分析2017年3月至2019年9月浙江省台州市中心医院(台州学院附属医院)胃肠肛肠外科收治的182例存在营养风险的胃肠道肿瘤手术患者的临床资料,根据营养支持情况分为规范营养支持组(118例)、不规范营养支持组(56例)和极不规范营养支持组(8例),比较分析三组患者术后的营养状况及临床结局。结果术后7d规范营养支持组患者的血清白蛋白、前白蛋白、转铁蛋白、血红蛋白水平及体重指数均高于不规范营养支持组和极不规范营养支持组(P<0.05)。规范营养支持组术后感染并发症及非感染并发症发生率(分别为7.62%、5.08%)均低于不规范营养支持组(分别为17.80%、8.92%)和极不规范营养支持组(分别为37.50%、25.00%),差异有显著性(P<0.05),而不规范营养支持组与极不规范营养支持组差异无显著性(P>0.05)。规范营养支持组住院时间短于不规范营养支持组和极不规范营养支持组(P<0.05),而不规范营养支持组与极不规范营养支持组差异无显著性(P>0.05)。规范营养支持组营养相关不良事件发生率(13.55%)与不规范营养支持组(16.07%)和极不营养支持组(25.00%)比较差异均无显著性(P>0.05)。结论规范营养支持可改善存在营养风险的胃肠道肿瘤患者围术期营养状况及临床结局。
Objective To investigate the effect of standardized nutritional support on the perioperative clinical outcome of patients with gastrointestinal cancer at nutritional risk.Method 182 cases of patients were included in the study,during March 2017 to September 2019 in the gastrointestinal surgical department of Taizhou center hospital(Taizhou university hospital).According to the standardability of nutritional support ways,divided into 3 groups,standard nutritional support group(118 cases),non-standard nutrition support group(56 cases),and extremely non-standard nutritional support group(8 cases).The nutritional status and clinical outcomes of 3 groups were compared and analyzed.Result The levels of serum albumin,prealbumin,transferrin,hemoglobin,and body mass index in the standard nutritional support group were higher than those in the non-standard nutritional support group and extremely non-standard nutritional support group(P<0.05).The incidence of postoperative infectious complications and non-infectious complications in the standard nutritional support group(7.62%and 5.08%,respectively)was lower than that in the non-standard nutritional support group(17.80%and 8.92%,respectively)and extremely non-standard nutritional support group(37.50%and 25.00%,respectively)(P<0.05),and there was no significant difference between the non-standard nutritional support group and the extremely non-standard nutritional support group(P>0.05).The length of hospital stay in the standard nutritional support group was shorter than that in the non-standard nutritional support group and the extremely non-standard nutritional support group(P<0.05),and there was no significant difference between the non-standard nutritional support group and the extremely non-standard nutritional support group(P>0.05).There was no significant difference in the incidence of nutrition-related adverse events in the standard nutritional support group(13.55%),non-standard nutritional support group(16.07%),and extremely poor nutritional support group(25.00%,P>0.05).Conclusion Standard nutritional support can improve perioperative nutritional status and clinical outcomes of patients with gastrointestinal cancer at nutritional risk.
