行为改变理论为框架的健康教育模式联合心理护理在烧伤患者治疗中的应用研究 被引量:11

摘要 目的:探究行为改变理论为框架的健康教育模式联合心理护理对烧伤患者的干预效果。方法:从2019年1月-2020年2月到笔者医院就诊的106例烧伤患者选为研究对象,根据护理方式将其分为观察组和对照组各53例。给予对照组烧伤整形术后常规康复护理,在对照组的基础上给予观察组行为改变理论为框架的健康教育模式联合心理护理干预。比较两组患者干预前及干预4周时的心理健康水平[心理韧性量表(CD-RISC)、体象障碍自评量表(BIRS)]、睡眠质量[匹兹堡睡眠质量指数(PSQI)]及疼痛情况(视觉模拟评分VAS);对两组患者随访6个月,比较其创伤后恢复状态,创伤后成长评定量表(PTGI)。结果:干预4周时,两组患者的CD-RISC坚韧性、力量性、乐观性评分较干预前明显提高,且观察组明显高于对照组,差异有统计学意义(P<0.05)。两组患者干预4周时的BIRS评分、PSQI评分、VAS评分较干预前明显降低,且观察组明显低于对照组,差异有统计学意义(P<0.05)。两组患者干预4周时的睡眠质量评价及疼痛评价较干预前明显改善,且观察组的睡眠质量(Z=2.252,P=0.024)及疼痛程度(Z=2.164,P=0.031)显著优于对照组。随访6个月时,观察组患者的PTGI评分(个人力量、人际关系、新的可能性、生活欣赏、精神变化)显著高于对照组,差异有统计学意义(P<0.05)。结论:行为改变理论为框架的健康教育模式联合心理护理对烧伤患者应用效果良好,该护理模式可有效改善患者的心理韧性及睡眠质量,促进其康复。 Objective To explore the intervention effect of health education model based on behavior change theory combined with psychological nursing on patients with burns.Methods 106 patients with moderate-to-severe burns who were treated in the hospital between January 2019 and February 2020 were selected as the research subjects,and were divided into observation group and control group according to the nursing modes,with 53 cases in each group.The control group was given routine rehabilitation nursing after burn plastic surgery,and on the basis of the control group,the observation group was given health education model based on behavior change theory combined with psychological nursing intervention.The psychological health level[Connor-Davidson Resilience Scale(CD-RISC),Body Image Rating Scale(BIRS)],sleep quality[Pittsburgh Sleep Quality Index(PSQI)]and pain(Visual Analogue Scale VAS)were compared between the two groups of patients before intervention and at 4 weeks of intervention.The two groups of patients were followed up for 6 months,and the post-traumatic recovery status[Post-Traumatic Growth Inventory(PTGI)]was compared.Results At 4 weeks of intervention,the scores of toughness,strength and optimism of CD-RISC scale of the two groups of patients were significantly improved compared with those before intervention,and the scores of observation group were significantly higher than those of control group(P<0.05).The BIRS score,PSQI scores and VAS score at 4 weeks of intervention in the two groups were significantly decreased compared to before intervention,and the scores of observation group were significantly lower than those of control group(P<0.05).The sleep quality evaluation and pain evaluation of the two groups of patients at 4 weeks of intervention were significantly improved compared to before intervention,and the sleep quality(Z=2.252,P=0.024)and pain degree(Z=2.164,P=0.031)of observation group were significantly better than those of control group.At 6-month follow-up,the PTGI scores(personal strength,interpersonal relationships,new possibilities,life appreciation,spiritual changes)in observation group were significantly higher than those in control group(P<0.05).Conclusion Health education model based on behavior change theory combined with psychological nursing has a good application effect on patients with burns.This nursing model can effectively improve the psychological resilience and sleep quality,and promote the rehabilitation of patients.
