摘要
目的探讨病毒性心肌炎(VMC)患儿血清正五聚体蛋白-3(PTX-3)及白细胞介素(IL)-35水平及其与心功能、心肌损伤的相关性。方法选取该院儿科收治的VMC患儿80例作为VMC组,选取同期于该院进行体检的50例健康儿童作为对照组。采用酶联免疫吸附试验比较两组血清中PTX-3、IL-35水平,采用放射免疫法检测VMC组患儿心肌酶谱表达,采用超声检测VMC组患儿心输出量(CO)、每搏输出量(SV)、射血分数(EF),采用Pearson相关分析血清PTX-3、IL-35水平与心肌酶谱及心功能的相关性。结果与对照组比较,VMC组患儿血清PTX-3水平升高,IL-35水平降低(P<0.05)。血清PTX-3水平与心肌肌钙蛋白I(cTnI)、肌酸激酶(CK)、肌酸磷激酶同工酶(CK-MB)、α-羟丁酸脱氢酶(α-HBDH)、天门冬氨酸氨基转移酶(AST)、乳酸脱氢酶(LDH)水平均呈正相关(r=0.816、0.671、0.861、0.869、0.913、0.603,P<0.05),与CO、SV、EF均呈负相关(r=-0.880、-0.836、-0.745,P<0.05)。血清IL-35水平与cTnI、CK、CK-MB、α-HBDH、AST、LDH均呈负相关(r=-0.539、-0.554、-0.589、-0.619、-0.531、-0.511,P<0.05),与CO、SV、EF均呈正相关(r=0.588、0.531、0.367,P<0.05)。结论VMC患儿血清中PTX-3水平升高,IL-35水平降低,且均与心肌损伤程度及心功能有相关性,PTX-3、IL-35水平检测对诊断VMC有一定的临床参考价值。
Objective To investigate the serum pentameric protein-3(PTX-3)and interleukin(IL)-35 levels in children with viral myocarditis(VMC)and their correlation with cardiac function and myocardial injury.Methods A total of 80 children with VMC admitted to the pediatrics department of the hospital were selected as the VMC group,and 50 healthy children who underwent physical examination in the hospital during the same period were selected as the control group.The serum levels of PTX-3 and IL-35 in the two groups were compared by enzyme-linked immunosorbent assay,and the expression of myocardial enzymes in the VMC group was detected by radioimmunoassay.Cardiac output(CO),stroke volume(SV)and ejection fraction(EF)in VMC group was detected by ultrasound.The correlation of serum PTX-3 and IL-35 levels with myocardial enzymes and cardiac function was analyzed by Pearson correlation.Results Compared with the control group,serum PTX-3 was increased and IL-35 was decreased in the VMC group(P<0.05).Serum PTX-3 levels were positively correlated with cardiac troponin I(cTnI),creatine kinase(CK),creatine phosphokinase isoenzyme(CK-MB),alpha-hydroxybutyrate dehydrogenase(α-HBDH),aspartate aminotransferase(AST),lactate dehydrogenase(LDH,r=0.816,0.671,0.861,0.869,0.913,0.603,P<0.05),and negatively correlated with CO,SV and EF(r=-0.880,-0.836,-0.745,P<0.05).Serum IL-35 levels were negatively correlated with cTnI,CK,CK-MB,α-HBDH,AST and LDH(r=-0.539,-0.554,-0.589,-0.619,-0.531,-0.511,P<0.05),and positively correlated with CO,SV,and EF(r=0.588,0.531,0.367,P<0.05).Conclusion The level of PTX-3 in the serum of children with VMC is increased,and the level of IL-35 is decreased,and both are related to the degree of myocardial injury and cardiac function.The detection of PTX-3 and IL-35 levels has a certain clinical reference value for the diagnosis of VMC.
