氢溴酸槟榔碱抗胃黏膜损伤的作用及机制 被引量:4

摘要 目的:探讨氢溴酸槟榔碱抗胃黏膜损伤的作用及其机制。方法:选取50只SPF级SD大鼠随机分为五组,每组10只,分别为空白对照组、生理盐水对照组、氢溴酸槟榔碱低剂量组、氢溴酸槟榔碱中剂量组及氢溴酸槟榔碱高剂量组(分别给予11 mg/kg、22 mg/kg、44 mg/kg氢溴酸槟榔碱)。取40只以乙酸灼烧法复制实验性胃溃疡模型,建模成功后用不同剂量的槟榔碱灌胃治疗7 d,收集胃液,用酸碱中和滴定法测量胃液游离酸量、总酸量,用胃蛋白酶检测试剂盒测定胃蛋白酶活性。后处死切开胃壁,观察大鼠胃黏膜损伤情况,并用游标卡尺测量溃疡面积,计算溃疡指数和溃疡抑制率。取胃窦部组织进行后固定,石蜡包埋,切片,HE染色,光学显微镜观察胃黏膜的病理变化。所得数据进行统计学分析。结果:氢溴酸槟榔碱剂量越高则大鼠胃溃疡指数越小,溃疡抑制率越大;氢溴酸槟榔碱高剂量组与对照组比较,溃疡指数明显变小,差异有统计学意义(P<0.01);形态学观察,生理盐水对照组黏膜损伤严重,黏膜上皮和固有层均出现变性坏死和较大的溃疡灶;氢溴酸槟榔碱低剂量组胃黏膜依然损伤严重,与生理盐水对照组差别不大;氢溴酸槟榔碱中剂量组胃黏膜变性坏死有所减轻,溃疡灶不明显;氢溴酸槟榔碱高剂量组胃黏膜损伤明显改善,上皮和固有层都有明显修复。中高剂量氢溴酸槟榔碱可减少胃游离酸量及总酸量;高剂量氢溴酸槟榔碱能明显抑制大鼠胃蛋白酶的活力,差异有统计学意义(P<0.05)。结论:氢溴酸槟榔碱能对抗大鼠胃黏膜发生损伤,并促进大鼠胃黏膜损伤后的修复,可能与其抑制胃酸的分泌及降低胃蛋白酶的活力有关。 Objective To investigate the effect and mechanism of arecoline hydrobromide on injury of gastric mucosa.Method 50 SPF SD rats were randomly divided into 5 groups,10 rats in each group.Blank control group,normal saline control group,low dose arecdine hydrobromide group,medium dose arecoline hydrobromide group and high dose arecoline hydrobromide group(11 mg/kg,22 mg/kg,44 mg/kg arecoline hydrobromoate,respectively).The experimental gastric ulcer model of 40 rats was made by acetic acid burning method.After successful modeling,the rats were treated intragastric with different doses of arecoline for 7 days.The gastric juice was collected,the free acid and total acid of gastric juice were measured by acid-base neutralization titration method,and pepsin activity was measured by pepsin detection kit.After the rats were sacrificed,the gastric wall was cut open to observe the gastric mucosa injury,and the ulcer area was measured with vernier calipers,and the ulcer index and ulcer inhibition rate were calculated.The gastric antrum tissue was taken for post-fixation,paraffin embedding,sectioning,HE staining,and the pathological changes of gastric mucosa were observed under light microscope.The obtained data were analyzed statistically.Results The higher the arecoline hydrobromide dose,the lower the gastric ulcer index and the higher the ulcer inhibition rate.Compared with the control group,the ulcer index of the high dose arecoline hydrobromoate group decreased significantly(P<0.01);Morphological observation showed that the mucosa of normal saline control group was severely damaged,and both the epithelium and lamina propria showed degeneration necrosis and large ulceration foci.Low dose arecoline hydrobromide group still had severe gastric mucosa injury,and there was no significant difference comparison of normal saline control group.In Medium dose arecoline hydrobromide group.the gastric mucosa degeneration and necrosis were alleviated and ulcer foci were not obvious.The gastric mucosa injury of the high dose arecoline hydrobromide group was significantly improved,and the epithelial and lamina propria were significantly repaired.Medium and high doses of arecoline hydrobromate could reduce the amount of free acid and total acid in the stomach.High dose of arecoline hydrobromoate significantly inhibited the activity of pepsin in rats,with statistical significance(P<0.05).Conclusion Arecoline hydrobromoate can inhibit gastric mucosa injury and promote the repair of gastric mucosa injury in rats,which may be related to the inhibition of gastric acid secretion and the decrease of pepsin activity.
