摘要
目的探究剪切波弹性成像(SWE)的弹性指数预测乳头状甲状腺癌颈部淋巴结转移的价值。方法选取2019年1月至2020年12月本院收治的84例乳头状甲状腺癌患者为研究对象,依据手术病理检查结果分为转移组(中央区淋巴结转移、颈侧区淋巴结转移,n=24)、非转移组(无颈部淋巴结转移,n=60)。两组均实施SWE检查,评价SWE的弹性指数(Emean、Emax、Emix)对患者颈部淋巴结转移的预测价值。结果转移组Emean、Emax、Emix均高于非转移组,且颈侧区淋巴结转移Emean、Emax、Emix水平均高于中央区淋巴结转移(P<0.05);ROC曲线分析表明,Emean、Emax、Emix临界点分别为126、85、26 kPa时,对乳头状甲状腺癌颈部淋巴结转移预测价值较高,其中Emean预测准确性最高(P<0.05)。结论利用SWE的弹性指数预测乳头状甲状腺癌颈部淋巴结转移效果理想,可作为临床诊断的参考依据。
Objective To investigate the value of shear wave elastography(SWE) elasticity index in predicting cervical lymph node metastasis of papillary thyroid cancer. Methods 84 patients with papillary thyroid cancer admitted to our hospital from January 2019 to December 2020 were selected as study subjects, they were divided into the metastatic group(central district lymph node metastasis, lateral cervical lymph node metastasis,n=24) and the non-metastatic group(no cervical lymph node metastasis, n=60). SWE was performed in both groups to evaluate the predictive value of SWE elasticity index(Emean, Emax, Emix) for cervical lymph node metastasis. Results The levels of Emean, Emax and Emix in metastatic group were higher than those in non metastatic group, and the levels of Emean, Emax and Emix in cervical lymph node metastasis were higher than those in central lymph node metastasis(P<0.05);ROC curve analysis showed that the critical points of Emean, Emax and Emix were 126, 85 and 26 kPa, respectively, which had higher predictive value for cervical lymph node metastasis of papillary thyroid cancer, and the predictive accuracy of Emean was the highest(P<0.05). Conclusion Using the elasticity index of swe to predict cervical lymph node metastasis of papillary thyroid cancer is ideal,which can be used as reference for clinical diagnosis.
