摘要
功能性胃肠病(FGIDs)症状重叠现象在临床上非常普遍,增加了临床诊治的难度。调中复衡理论是在继承脾胃学说和通降论学术思想的基础上,结合长期临床实践所构建的脾胃病治疗理论。基于调中复衡理论,认为脾胃功能失调、气机升降失衡是FGIDs症状重叠现象发生的核心病机,其治疗目标是恢复气机升降平衡及脏腑功能平衡的"两平衡"状态,辨治则以"八纲通胃法"作为具体方法,即以脏腑为纲、多脏腑同调为主,以气血为纲、调气复通降为要,以虚实为纲、补虚泻实为法,以寒热为纲、平调寒热为常。以调中复衡理论为指导,纲目分明,执简驭繁,可为临床诊治FGIDs症状重叠厘清思路。
Symptom overlap in functional gastrointestinal disorders(FGIDs)is frequetly occured,which increases the difficulty of clinical diagnosis and treatment.The theory of regulating the middle and restoring the balance is generated based on the inheritance of the spleen-stomach theory and unblocking and purging theory and on the accumulation of long-term clinical practice.According to this theory,the spleen and stomach dysfunction and the imbalance of qi movement are the core pathogenesis of FGIDs symptom overlap,and the treatment goal is to restore the dual balance of qi movement and zang-fu(脏腑)organs function.The method of“unblocking the stomach with eight-principle pattern differentiation”is suggested;in terms of zang-fu principle,the combined regulation of multiple zangfu organs is set as the focus;for qi-blood principle,it is critical to regulate qi and restore unblocking and purging;guided by the deficiency-excess principle,it is suggested to supplement deficiency and drain the excess;following the cold-heat principle,it is usually to regulate cold and heat with even methods.The theory of regulating the middle and restoring the balance has clear principles and makes it easier to be mastered,providing references for clinical diagnosis and treatment of FGIDs symptom overlap.
