摘要
目的:探究血液透析患者营养状态与机体炎性反应状态的关系。方法:选取2019年1月-2021年7月福建医科大学附属协和医院的96例血液透析患者为观察组,同时期的96名健康者为对照组。比较两组的血清营养状态相关指标(血清ALB、Hb、PA、SF)、PG-SGA评分及炎性反应指标[白细胞介素-1β(IL-1β)、IL-6、IL-10、CRP、TNF-α],比较不同PG-SGA评分血液透析者的血清营养状态相关指标及炎性反应指标,采用Pearson相关性分析血清营养状态相关指标与炎性反应指标的关系,采用Spearman秩相关分析炎性反应指标与PG-SGA评分的关系。结果:观察组的血清营养状态相关指标均显著低于对照组,PG-SGA评分及炎性反应指标均显著高于对照组,不同PG-SGA评分血液透析者的血清营养状态相关指标及炎性反应指标比较,差异均有统计学意义(P<0.05);Pearson相关性分析显示,血清营养状态相关指标与炎性反应指标均呈负相关(P<0.05);Spearman秩相关分析显示,炎性反应指标与PG-SGA评分均呈正相关(P<0.05)。结论:血液透析患者的营养状态与机体炎性反应状态显著异常,且营养状态评估指标与机体炎性反应状态指标密切相关,因此在血液透析患者中的评估与检测价值均较高。
Objective:To investigate the correlation between nutritional status and body inflammatory reaction status of patients with hemodialysis.Method:A total of 96 patients with hemodialysis in Fujian Medical University Union Hospital from January 2019 to July 2021 were chosen as the observation group,96 healthy persons at the same time were chosen as the control group.Then the serum nutritional status related indexes (serum ALB,Hb,PA,SF),PG-SGA score and inflammatory reaction indexes[interleukins-1β (IL-1β),IL-6,IL-10,CRP,TNF-α]of two groups were compared,then the serum nutritional status related indexes and inflammatory reaction indexes of patients with different PG-SGA score of hemodialysis were compared too,and the correlation between the serum nutritional status related indexes and inflammatory reaction indexes were analyzed with Pearson correlation analysis,the correlation between inflammatory reaction indexes and PG-SGA score were analyzed with Spearman rank correlation analysis.Result:The serum nutritional status related indexes of observation group were significantly lower than those of control group,and the PG-SGA score and inflammatory reaction indexes were significantly higher than those of control group,the serum nutritional status related indexes and inflammatory reaction indexes of patients with different PG-SGA score of hemodialysis were compared,there were statistically significant differences (P<0.05);the Pearson correlation analysis showed the serum nutritional status indexes were negatively correlated with inflammatory reaction indexes (P<0.05);the Spearman rank correlation analysis showed the inflammatory reaction indexes were positively correlated with PG-SGA score (P<0.05).Conclusion:The nutritional status and inflammatory reaction status of patients with hemodialysis are significantly abnormal,and the evaluation indexes of nutritional status are closely related to the indexes of inflammatory response,so the evaluation and detection value in the patients with hemodialysis are higher.
