摘要
目的:研究血清同型半胱氨酸(Hcy)、胃蛋白酶原(PG)在慢性萎缩性胃炎(CAG)患者中的表达及影响因素。方法:将本院从2018年4月-2020年4月收治的72例CAG患者纳入研究,记作研究组;另取同期健康体检者70例作为对照组。检测并比较两组血清Hcy和PG相关指标水平、基线资料。并以多因素logistic回归分析明确CAG发生的影响因素。此外,将CAG患者按照病情程度的差异分为一般组(n=32)及严重组(n=40),分析两组血清Hcy和PG的差异。结果:研究组血清Hcy、PGⅡ水平均高于对照组,而PGⅠ水平低于对照组(P<0.05)。研究组吸烟、饮酒以及Hp感染阳性例数占比均高于对照组(P<0.05)。多因素logistic回归分析显示,血清Hcy、PGⅡ以及吸烟、饮酒、Hp阳性均是CAG的独立危险因素,而PGⅠ为保护性因素(P<0.05)。严重组患者的血清Hcy、PGⅡ水平均高于一般组,而PGⅠ水平低于一般组(P<0.05)。结论:CAG患者血清Hcy、PGⅡ水平均较高,PGⅠ水平较低;发生CAG的影响因素较多,需要临床重点关注。
Objective:To study the expression and influencing factors of serum homocysteine (Hcy) and pepsinogen (PG) in patients with chronic atrophic gastritis (CAG).Method:A total of 72 patients with CAG admitted to our hospital from April 2018 to April 2020 were included in the study as study group,another 70 healthy subjects were selected as the control group.Serum Hcy and PG levels and baseline data were detected and compared between the two groups,the influencing factors of CAG were determined by multivariate logistic regression analysis.In addition,CAG patients were divided into the general group (n=32) and the severe group (n=40) according to the difference of disease severity,and the differences of serum Hcy and PG between the two groups were analyzed.Result:Serum Hcy and PGⅡ levels in the study group were higher than those in the control group,while PGⅠ level was lower than that of the control group (P<0.05).The proportion of smoking,drinking and Hp infection positive in the study group were higher than those in the control group (P<0.05).Multivariate logistic regression analysis showed that serum Hcy,PGⅡ,smoking,drinking and Hp positive were independent risk factors for CAG,while PGⅠ was protective factor (P<0.05).Serum Hcy and PGⅡ levels in severe group were higher than those in general group,while PGⅠ level was lower than that in general group (P<0.05).Conclusion:Serum Hcy and PGⅡ levels are higher in CAG patients,but PGⅠ levels are lower;there are many influencing factors for the occurrence of CAG,which requires clinical attention.
