摘要
目的探析双镜联合钬激光治疗肝内外胆管结石疗效及安全性。方法选取2019年1月—2021年1月期间河源长安医院接受治疗的肝内外胆管结石患者66例作为研究对象,按计算机分组法随机分为3组,其中20例患者采用开腹下胆总管切开探查取石术,列为开腹组;23例采用采用腹腔镜联合胆管镜下胆总管探查取石术治疗,列为腔镜组;23例患者采用腹腔镜联合胆管镜钬激光碎石手术治疗,列为钬激光组;统计并对比3组患者围术期指标[手术时间、术中出血量、术后排气时间],及并发症发生情况。结果钬激光组患者的手术时间、术中出血量、术后排气时间均低于腔镜组和开腹组,其术后并发症发生率8.70%(2/23)也低于腔镜组21.74%(5/23)和开腹组35.00%(7/20),差异均有统计学意义(P<0.05)。结论相较于常规开腹探查取石及腹腔镜下微创取石术,双镜联合钬激光取石术治疗肝内外胆管结石的疗效更佳,可显著缩短患者的手术时间及术后排气时间并减少术中出血量,且并发症发生风险更低,可在临床进一步推广应用。
Objective Analyze the effectiveness and safety of dual wells combined with holmium laser in primary hospitals in the treatment of patients with intra-and extra-hepatic bile duct stones.Methods A total of 66 patients with intrahepatic and extrahepatic bile duct stones who were treated in our hospital from January 2019 to January 2021 were selected as the research objects. All patients were randomly divided into three groups according to the computerized grouping method,and they were treated with different surgical methods. Statistics and statistics The perioperative indicators(operating time,intraoperative blood loss,postoperative exhaust time)and complications of the three groups were compared.Results The operation time,intraoperative blood loss,and postoperative exhaust time of the holmium laser group were lower than those of the laparoscopy group and the laparotomy group,and the postoperative complications rate of 8.70%(2/23)was also lower than that of the laparoscopy group 21.74 %(5/23)and 35.00%(7/20) in the open group(P<0.05).Conclusion Compared with the conventional laparotomy and laparoscopic minimally invasive lithotripsy,the double mirror combined with holmium laser lithotripsy is more effective in the treatment of patients with intrahepatic and extrahepatic bile duct stones,which can significantly shorten the patient’s operation time and postoperative exhaust. It takes time and reduces the amount of blood loss during the operation,and the risk of postoperative complications is lower after the double mirror combined with holmium laser treatment. The effectiveness and safety of this operation are higher. It is recommended that it can be further promoted and applied in the clinic.
