摘要
目的探讨布地奈德、异丙托溴铵联合沙丁胺醇雾化治疗小儿支气管肺炎的临床效果。方法选取2020年8月至2021年8月我院收治的200例小儿支气管肺炎患儿作为研究对象,将其随机分为对照组(100例,布地奈德+异丙托溴铵)与观察组(100例,布地奈德+异丙托溴铵+沙丁胺醇)。比较两组的治疗效果。结果观察组的治疗总有效率为98.00%,高于对照组的90.00%(P<0.05)。观察组的退热时间、肺部啰音消失时间、X线肺部阴影消失时间及咳嗽消失时间均短于对照组(P<0.05)。治疗后,两组FEV;、FVC及PEF均较治疗前增大,且观察组大于对照组(P<0.05)。治疗后,两组的PaCO;均较治疗前降低,PaO;、SaO;均较治疗前升高,且观察组明显优于对照组(P<0.05)。治疗后,两组的日间咳嗽评分与夜间咳嗽评分均较治疗前降低,且观察组低于对照组(P<0.05)。治疗后,两组的WBC、hs-CRP、PCT、MCP-4、MDC、ECP水平均较治疗前降低,且观察组低于对照组(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论布地奈德、异丙托溴铵与沙丁胺醇联合治疗小儿支气管肺炎的疗效显著,可缩短患儿的症状及体征改善时间,改善患儿的肺功能、血气指标、实验室指标及趋化因子水平。
Objective To investigate the clinical effect of budesonide, ipratropium bromide combined with salbutamol atomization in the treatment of bronchopneumonia in children. Methods Two hundred children with bronchopneumonia treated in our hospital from August 2020 to August 2021 were randomly divided into control group(100 cases, budesonide+ ipratropium bromide) and observation group(100 cases, budesonide + ipratropium bromide + salbutamol). The therapeutic effects of the two groups were compared. Results The total effective rate of treatment in the observation group was 98.00%,which was higher than 90.00% in the control group(P<0.05). The antipyretic time, lung rale disappearance time, X-ray lung shadow disappearance time and cough disappearance time in the observation group were shorter than those in the control group(P <0.05). After treatment, FEV;, FVC and PEF in the two groups increased compared with those before treatment, and those in the observation group were greater than the control group(P<0.05). After treatment, the PaCO;in the two groups was lower than that before treatment, and the PaO;and SaO;were higher than those before treatment, and those in the observation group were significantly better than the control group(P<0.05). After treatment, the daytime cough score and night cough scores in the two groups were lower than those before treatment, and those in the observation group were lower than the control group(P<0.05). After treatment, the WBC, levels of hs-CRP, PCT, MCP-4, MDC and ECP in the two groups were lower than those before treatment, and those in the observation group were lower than the control group(P<0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05). Conclusion The regime budesonide, ipratropium bromide and salbutamol is effective in the treatment of children with bronchopneumonia. it can shorten the improvement time of symptoms and signs, and improve the pulmonary function,blood gas indexes, laboratory indexes and chemokine levels of children.
