摘要
目的分析经颅多普勒超声(TCD)联合彩色多普勒超声血流显像(CDFI)在颈动脉狭窄患者内膜剥脱术(CEA)治疗中的应用价值。方法回顾性收集南阳南石医院2019年1月至2020年7月在CEA治疗前后采用CDFI检测的41例颈动脉狭窄患者临床资料,纳入对照组;另将同期采用在CEA治疗前后采用TCD联合CDFI检测的41例颈动脉狭窄患者临床资料,纳入观察组;比较2组患者术前病变检测的准确率,并比较2组患者术后24 h时血液流通状态[狭窄段峰值流(PSV)、舒张末期流速(EDV)、平均血流速(MV)、血管搏动指数(PI)]。结果术前,观察组病变检出率和对比血管造影准确率高于对照组,差异有统计学意义(P<0.05);CEA术后24 h,观察组MV、PSV、EDV、PI均高于对照组,差异有统计学意义(P<0.05)。结论 TCD联合CDFI在颈动脉狭窄患者CEA治疗中的应用效果显著,可有效提高病变检测准确率,改善血液流通状态。
Objective To explore the application value of transcranial Doppler(TCD) combined with color Doppler flow imaging(CDFI) in the treatment of carotid artery stenosis patients with carotid endarterectomy(CEA).Methods the clinical data of 41 patients with carotid artery stenosis before and after CEA treatment in Nanyang Nanshi hospital from January 2019 to July 2020 were retrospectively analyzed. In addition, the clinical data of 41 pa-tients with carotid artery stenosis detected by TCD combined with CDFI before and after CEA treatment were included in the observation group. The accuracy of preoperative lesion detection was compared between the two groups, and the blood flow status [peak stenotic flow(PSV), end diastolic flow(EDV), mean blood flow(MV), and vascular pulsatility index(PI)] at 24 hours after operation were compared between the two groups. Results before operation, the detection rate of lesions and the accuracy of contrast angiography in the observation group were higher than those in the control group, and the differences were statistically significant(P<0.05);24 hours after CEA, MV, PSV, EDV and PI of the observation group were higher than those of the control group, and the differences were statistically significant( P<0.05). Conclusion TCD combined with CDFI in the treatment of carotid artery stenosis in patients with CEA application effect is significant, can effectively improve the accuracy of disease detection, improve blood flow status.
