摘要
目的:研究低气腹压联合腹壁悬吊对60~70岁腹腔镜胆囊切除术患者循环功能、眼内压及纤溶指标水平的影响。方法:选取2018年10月至2020年10月期间在我科接受胆囊切除术的125例老年患者作为研究对象。按照随机数字法分为对照组(n=62)和观察组(n=63)。其中对照组采用常规气腹压干预,观察组采用低气腹压联合腹壁悬吊干预,对比两组患者的循环功能、眼内压和纤溶指标。结果:观察组患者干预后平均动脉压(Mean arterial pressure,MAP)、心率(Heart rate,HR)、双侧眼内压(Intraocular pressure,IOP)、纤溶酶原激活物(t-Plasminogen activator,t-PA)和纤溶酶原激活物抑制物-1(Plasminogen activator inhibitor-1,PAI-1)均明显低于对照组(P<0.05)。结论:低气腹压联合腹壁悬吊对60~70岁腹腔镜胆囊切除术患者实施干预后,可以提高患者循环功能水平,降低眼内压,优化纤溶指标水平。
Objective:To investigate the effects of low pneumoperitoneum pressure combined with abdominal wall suspension on circulatory function,intraocular pressure and fibrinolytic indexes in 60~70 years elderly patients undergoing laparoscopic cholecystectomy.Methods:A total of 125 elderly patients who underwent cholecystectomy in our department from October 2018 to October 2020 were selected as the research objects.According to random number method,they were divided into control group(n=62)and observation group(n=63).The control group was treated with conventional pneumoperitoneum pressure intervention,and the observation group was treated with low pneumoperitoneum pressure combined with abdominal wall suspension intervention.The circulatory function,intraocular pressure and fibrinolytic indexes of the two groups were compared.Results:After intervention,mean arterial pressure(MAP),heart rate(HR),and intraocular pressure(Intraocular pressure)in observation group were observed.Intraocular pressure(IOP),T-plasminogen activator(T-PA)and plasminogen activator inhibitor-1(PAI-1)were significantly lower than those in control group(P<0.05).Conclusion:The intervention of low pneumoperitoneum pressure combined with abdominal wall suspension in 60~70 years elderly patients with laparoscopic cholecystectomy can improve the level of circulation function,reduce intraocular pressure,and optimize fibrinolysis index.
