摘要
目的明确单纯性和合并不同类型感染的混合性滴虫阴道炎在滴虫性阴道炎中所占比例,为临床合理用药及诊治该病提供科学依据。方法收集北京地区某三甲医院妇产科2020全年12个月中所有滴虫性阴道炎患者的白带分泌物标本,通过革兰氏染色进行念珠菌及细菌的检测;用盐水悬滴法检查滴虫活动力,将上述结果结合发病季节及患者年龄进行统计分析。结果全年共134例患者被诊断为滴虫性阴道炎,其中单纯性滴虫感染患者为54例(40.3%),混合性滴虫阴道炎为80例(59.7%)。混合性阴道炎中合并细菌感染的为75例(56.0%)。所有患者中,白带分泌物中白细胞Ⅲ度及以上发生率为80%,上皮细胞Ⅱ度及以上发生率为20%。结论混合性滴虫阴道炎发病率高于单纯性,合并细菌感染为主要混合性阴道炎的发病群体。滴虫感染发生率与季节和年龄有相关性。滴虫感染患者的阴道菌群大多数为失衡状态。临床应根据不同感染情况制定相应的治疗及预防措施。
Objective To determine the ratio of simple and mixed trichomonas vaginitis with different types of infection in trichomonas vaginitis, and to provide a scientific basis for clinical rational drug use and diagnosis and treatment of this disease.Methods Leucorrhea secretion samples were collected from all patients with trichomonas vaginitis in the Department of Obstetrics and Gynecology of a grade A hospital in Beijing for 12 months in 2020. The candida and bacteria were detected by Gram staining. The activity of trichomonas was examined by saline suspension drop method. We then combined these data with the data of onset season and age of these patients and conducted a statistical analysis.Results A total of 134 patients were diagnosed with trichomonas vaginitis, including 54(40.3%) patients with simple trichomonas infection and 80(59.7%) patients with mixed trichomonas vaginitis. There were 75 cases(56.0%) of mixed vaginitis with bacterial infection. In all patients, the incidence of grade Ⅲ or above in leucorrhea secretions was 80%, and the incidence of grade Ⅱ or above in epithelial cells was 20%.Conclusion The incidence of mixed trichomonas vaginitis is higher than that of simple vaginitis, and bacterial infection is the main group of mixed vaginitis. The incidence of trichomonas infection is correlated with season and age. Most of the vaginal microflora of trichomonas infected patients is unbalanced. Clinical treatment and prevention measures should be made according to different infection conditions.
