摘要
目的比较注意缺陷多动障碍(ADHD)三种亚型儿童的行为特征.方法采用美国精神障碍诊断与统计手册第五版(DSM-Ⅴ)诊断标准和《斯诺佩评估量表》(SNAP-Ⅳ),将130例ADHD患儿分成ADHD-I型、ADHD-C和ADHD-HI型.采用儿童行量表(CBCL)和Conners父母症状问卷(PSQ)比较各亚型和正常儿童的行为问题.结果(1)PSQ量表中:ADHD亚型与对照组比较,除心身障碍分项中差异无统计学意义,其余分项中差异均有统计学意义.CBCL量表中,ADHD亚型与对照组比较,除躯体主诉分项中差异无统计学意义,其余分项差异均有统计学意义.(2)PSQ量表中:学习问题方面,ADHD-I和ADHD-C型均高于ADHD-HI型和对照组;品行问题方面,ADHD-C型高于ADHD-I、ADHD-HI型和对照组.焦虑因子方面,ADHD-I型高于对照组和ADHD-HI型.在多动/冲动因子,ADHD-C型和ADHD-HI型高于ADHD-I型和对照组.CBCL两两比较中,ADHD-C在行为问题分项、注意力分项高于ADHD-I、ADHD-HI和对照组;ADHD-C在外化性行为问题、攻击行为分项中高于ADHD-I;ADHD-I在退缩分项中高于ADHD-HI和对照组.结论ADHD临床亚型在行为问题上存在差异,混合型问题最突出;而多动冲动型仅在多动/冲动领域存在问题.
Objective To compare the behavioral features of different subtypes of attention deficit/hyperactivity disorder(ADHD).Methods Use DSM-Ⅴ criteria and SNAP-Ⅳ to divide 130 children with ADHD into 3 subtypes:inattentive type(ADHD-I),the predominantly hyperactive/impulsive type(ADHD-HI),and the combined type(ADHD-C).Adopt Child Behavior Checklist(CBCL)and Parent symptom questionnaire(PSQ)to compare behavior problem of different subtypes of ADHD and normal children.Results(1)There were significant differences between 3 subtypes of ADHD and control group in all sub-scale except psychosomatic disorder sub-scale in PSQ and Somatic complaints subscale in CBCL.(2)Through post hoc test of PSQ,ADHD-I and ADHD-C showed higher score than ADHD-HI and control group in learning problem sub-scale;ADHD-C showed higher score than the remaining 3 groups in conduct problem sub-scale;ADHD-I scored higher than ADHD-HI and control group in anxiety-depression sub-scale;ADHD-C and ADHD-HI scored higher than ADHD-I and control group.Through post hoc test of CBCL,ADHD-C scored higher than the other 3 groups in conduct and attention problem sub-scales.ADHD-C showed higher score in external behavior and aggressive behavior than ADHD-I;ADHD-I scored higher in recession sub-scale than ADHD-HI and control group.Conclusion The characteristics of behaviors are different among three subtypes of ADHD.The behavior problem of ADHD-C is the most prominent.And ADHD-HI has only manifest hyperactive/impulsive problems.
