摘要
目的探究X射线调强放疗与电子线放疗对早期乳腺癌手术患者血清肿瘤标志物、T细胞亚群及副反应的影响。方法选取行X射线调强放疗的早期乳腺癌术后患者48例为观察组,同期行电子线放疗的早期乳腺癌术后患者42例为对照组,2组患者术后均进行全乳照射、瘤体补量治疗,剂量均为10~16 Gy/5~8次;比较2组患者的照射剂量、血清肿瘤标志物[癌胚抗原(CEA)、糖类抗原15-3(CA153)]、T细胞亚群(CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+))及副反应。结果两组乳腺癌手术患者术后放疗靶区V 95%比较差异无统计学意义(P>0.05),但观察组靶区V 105%、V 110%量、心脏及患侧肺照射剂量低于对照组(P<0.05);放疗前2组患者CEA、CA153水平、CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)比较,差异无统计学意义(P>0.05);放疗后1月、3月血液检查结果显示,2组患者血液CEA、CA153、CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)水平均随放疗时间延长而降低,观察组CEA、CA153水平均低于对照组(P<0.05),CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)高于对照组(P<0.05);2组放疗毒副反应多集中发生在Ⅰ~Ⅱ级患者中,两组毒副反应比较差异无统计学意义(P>0.05)。结论X射线调强放疗可降低乳腺癌手术患者血清肿瘤标志物水平,对患者免疫功能影响小。
Objective To explore the effects of X-ray intensity modulated radiation therapy and electronic beam radiation therapy on serum tumor markers,T cell subsets,and side effects after early breast cancer surgery.Methods Forty-eight postoperative patients who underwent X-ray intensity modulated radiation therapy for early breast cancer were selected as the observation group,and 42 postoperative patients who underwent electronic beam radiation therapy for early breast cancer were selected as the control group.After operation,both groups were treated with whole breast irradiation and tumor replenishment therapy,and the amount was 10-16 Gy/5-8 times;exposure dose,serum tumor markers[cancerous embryonic antigen(CEA),sugar antigen 153(CA153)],T cells subsets(CD3^(+),CD4^(+),CD4^(+)/CD8^(+)),and side reactions were compared.Results There was no statistical significance in the difference in V 95%between the two groups(P>0.05),but the doses of target area V 105%,V 110%,heart and affected lung in observation group were lower than those in control group(P<0.05).There was no statistical significance in the differences in CEA,CA153 levels,CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)before radiotherapy(P>0.05);blood tests(1 month and 3 months after radiotherapy)showed that CEA,CA153 levels,CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)in both groups decreased with prolonged radiotherapy time,but CEA,CA153 levels in the observation group were lower than control group(P<0.05)and CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)were higher than control group(P<0.05);In both groups,radiotherapy side effects mainly existed in levelⅠand levelⅡpatients,and there were no statistical differences in side effects in both groups(P<0.05).Conclusion X-ray intensity modulated radiation therapy can reduce the level of serum tumor markers after breast cancer surgery,and have little impact on patients’immune function.
