摘要
目的通过二维斑点追踪成像(2D-STI)技术评价慢性肺源性心脏病(CPHD)伴有不同程度肺动脉高压(PAH)患者右心室收缩功能。方法收集2016年7月至2020年5月本院诊断为CPHD患者的临床资料。将入选的CPHD患者根据肺动脉收缩压(PASP)的不同分为3组:A组,PASP≥30~50 mmHg;B组,PASP≥50~70 mmHg;C组,PASP≥70 mmHg。剔除各组中图像质量差无法分析的患者,每组分别选取图像质量较好的患者22例。同期选取健康志愿者26例为对照组。对所有入选者进行2D-STI检查,测量各组各节段纵向收缩期峰值应变(S)及收缩期峰值应变率(SRs),对比各组二维斑点参数的差异。结果与对照组比较,A组、B组、C组右心室游离壁各节段S值均减低,且A组、B组、C组S值依次降低,组间比较差异有统计学意义(均P<0.05)。结论2D-STI可以敏感、准确评价CPHD伴有不同程度肺动脉高压患者右心功能受损情况。
Objective To evaluate right ventricular systolic function in chronic pulmonary heart disease(CPHD)patients with different degrees of pulmonary hypertension(PAH)using the two-dimensional speckle tracking imaging(2D-STI).Methods CPHD patients were collected from July 2016 to May 2020 in Shengjing Hospital of China Medical University.CPHD patients were divided into three groups according to differences in pulmonary artery systolic pressure(PASP).Patients with images of poor quality that could not be analyzed were excluded from each group,leading to the selection of 22 cases with good imaging quality from each group.Group A was the mild group(PASP≥30-50 mmHg),group B was the moderate group(PASP≥50-70 mmHg),and group C was the severe group(PASP≥70 mmHg).Meanwhile,26 healthy volunteers were selected to be part of control group.All participants were examined using 2D-STI.Longitudinal peak systolic strain(S)and peak systolic strain rate(SRs)were measured by 2D-STI in each group and segment.Differences between two-dimensional conventional parameters of ultrasound were compared.Results Compared with control group,the S of right ventricular free wall in group A,B,and C were decreased.The S of group A,B,and C decreased in turn.The difference between the groups was statistically significant(P<0.05).Conclusion 2D-STI can sensitively and accurately evaluate the impairment of right ventricular function in patients with CPHD with varying degrees of pulmonary hypertension.
