18F-FDG PET/CT检查联合血清VEGF、sFLT1、CD105检测预测子宫内膜癌浸润与转移的价值

Value of 18F-FDG PET/CT combined with serum VEGF,sFLT1,CD105 in predicting endometrial cancer infiltration and metastasis

摘要 目的探讨18氟-氟代脱氧葡萄糖-正电子发射计算机断层显像/电子计算机断层扫描(18F-FDG PET/CT)联合血清内皮生长因子(VEGF)、内皮生长因子受体-1(s FLT1)、内皮糖蛋白(CD105)预测子宫内膜癌浸润与转移的价值。方法选取南阳市某医院2019年1月—2020年6月诊治的72例子宫内膜癌患者为研究对象。按照病理诊断结果将患者分成对照组和研究组,对照组为未浸润和转移的子宫内膜癌患者,共42例;研究组为浸润和转移的子宫内膜癌患者,共30例。比较2组患者最大标准化摄取值(SUVmax)和平均标准化摄取值(SUVmean)、代谢活性肿瘤体积(MTV)和病灶糖酵解总量(TLG)及血清VEGF、s FLT1、CD105水平。结果研究组患者SUVmax、SUVmean、MTV和TLG值均小于对照组,差异均有统计学意义(P<0.05)。研究组患者血清VEGF、CD105水平高于对照组(P<0.05),而s FLT1水平低于对照组,差异均有统计学意义(P<0.05)。18F-FDG PET/CT+VEGF+s FLT1+CD105的曲线下面积(AUC)为0.956(95%CI:0.915~0.998)。18F-FDG PET/CT+VEGF+s FLT1+CD105的AUC最大,说明准确性最高。结论使用18F-FDG PET/CT影像学参数中SUVmax、SUVmean、MTV和TLG联合血清VEGF、s FLT1、CD105预测子宫内膜癌浸润与转移的价值高于单一使用影像学参数和血清学指标的预测价值。 Objective To investigate the value of 18F-fluordeoxyglucose positron emission tomography/co mputed tomography(18F-FDGPET/CT)imaging combined with serum vascular endothelial growth factor(VEGF),soluble fms-like tyrosine kinase-1(sFLT1),and endoglin(CD105)in predicting the value of endometrial cancer infiltration and metastasis.Methods Seventy-two patients with endometrial cancer treated from January 2019 to June 2020 in a hospital in Nanyang were selected for the study.Patients were divided into control group and study group according to pathological diagnosis.The control group were patients with non-infiltrating and non-metastatic endometrial cancer,totaling 42 cases;the study group were patients with infiltrating and metastatic endometrial cancer,totaling 30 cases.The maximum standardized uptake value(SUVmax)and mean standardized uptake value(SUVmean),metabolic tumor volume(MTV),total lesion glycolysis(TLG)and serum VEGF,sFLT1 and GD105 levels were compared between the 2 groups.Results The values of SUVmax,SUVmean,MTV and TLG in the study group were smaller than those in the control group,and the differences were all statistically significant(P<0.05).Serum VEGF and CD105 levels were higher in the study group than in the control group(P<0.05),while sFLT1 levels were lower than in the control group,and all differences were statistically significant(P<0.05).The area under curve(AUC)of 18F-FDG PET/CT+VEGF+sFLT1+CD105 was 0.956(95%CI:0.915-0.998).18 F-FDG PET/CT+VEGF+sFLT1+CD105 had the largest AUC,indicating the highest test accuracy.Conclusion The value of using SUVmax,SUVmean,MTV and TLG in combination with serum VEGF,sFLT1 and CD105 in 18F-FDG PET/CT imaging parameters to predict endometrial cancer infiltration and metastasis was higher than the predictive value of using imaging parameters and serological indicators alone.
