摘要
目的分析MRI定量分析髌股关节形态参数在诊断CP中的临床价值。方法选取2019年3月至2021年3月住院治疗的200例CP患者,根据“检查方法的不同”分为2组。观察1组100例患者采纳关节镜手术检查,观察2组100例患者采纳MRI检查,比较2组检出率、漏诊率、检出时间。选取2019年3月至2021年3月门诊接诊的100例无CP患者作为参照组,比较观察2组、参照组髌骨形态参数、股骨滑车形态参数。结果观察2组检出率(100.00%)、失误率(0)与观察1组(98.00%、2.00%)比较,差异无统计学意义(P>0.05)。检出时间观察2组比观察1组长(P<0.05)。观察2组髌骨I-S指数、髌骨倾斜角均高于观察1组,观察2组髌骨Wiberg指数低于观察1组(P<0.05);观察2组髌骨面角与观察1组比较,差异无统计学意义(P>0.05)。观察2组滑车深度、外侧滑车倾斜角、滑车内外侧关节面比值均低于观察1组,观察2组滑车沟角高于观察1组(P<0.05)。结论MRI定量分析髌股关节形态参数可对CP患者病情作出准确的评估与诊断,临床价值较高。
Objective To investigate the clinical value of quantitative analysis of morphological parameters of patellofemoral joint by magnetic resonance(MRI)in diagnosis of chondromalacia patellae(CP).Methods Two hundred patients with CP who were treated in our hospital from March 2019 to March 2021 were enrolled in the study,who were divided into two groups according to different examination methods,with 100 cases in each group.The patients in observation 1 group underwent arthroscope examination,and thepatients in observation 2 group underwent MRI examination.The detection rate,missed diagnosis rate,and detection time were observed and compared between the two groups.Moreover the other 100 patients without CP who were treated in our hospital from March 2019 to March 2021 were enrolled as control group.The patella morphological parameters and femoral trochlear morphological parameters were observed and compared among the three groups.Results There were no significant differences in the detection rate and error rate between observation 2 group and observation 1 group(P>0.05).The detection time,and patella I-S index and the patella inclination angle in observation 2 group were significantly higher than those in observation 1 group,and the Wiberg indexes of patella in observation 2 groups were significantly lower than those in observation 1 group(P<0.05).The pulley depth,lateral pulley inclination angle,and the ratio of the inner and outer articular surfaces of the pulley in observation 2 group were significantly lower than those in observation 1 group,however,the pulley groove angle in observation 2 group was significantly higher than that in observation 1 group(P<0.05).Conclusion MRI quantitative analysis of the morphological parameters of the patellofemoral joint can make accurate assessment and diagnosis for the disease condition of patients with CP,which has higher clinical value.
