以血便为首发症状的早产儿食物蛋白诱导的直肠结肠炎5例并文献复习 被引量:1

摘要 目的探讨早产儿食物蛋白诱导的直肠结肠炎(food protein-induced proctocolitis,FPIP)的临床特征,减少误诊误治。方法对北京大学深圳医院收治的以血便为首发症状的早产儿FPIP 5例进行回顾性分析。结合国内外数据库检索文献中的病例资料,分析总结早产儿FPIP的临床特征及研究进展。结果5例最初均诊断为坏死性小肠结肠炎,胎龄31^(+3)~33^(+6)周,发病时间生后3~39 d,血便程度从少量血丝便到大量黏液血便,其他症状包括呕吐、腹胀,肠鸣音均正常,4例伴嗜酸性粒细胞升高,4例感染指标正常,2例腹部平片可见肠壁积气,5例情况及预后良好。文献复习显示,临床特征与该研究基本相符,同时报道FPIP外周血及大便嗜酸性粒细胞增多,特应性斑贴试验、皮肤点刺试验可有阳性,肠道超声在FPIP仅局部肠袢受累,其余肠段可表现为蠕动正常甚至增加。结论对于早产儿血便,要警惕FPIP,特别是当临床征象与感染中毒症状、腹部体征、感染指标等不相称时。嗜酸性粒细胞增多、特应性斑贴试验、皮肤点刺试验及肠道超声等有助于诊断。
