纳洛酮联合高压氧治疗颅脑损伤的临床效果

Clinical effect of naloxone combined with hyperbaric oxygen in the treatment of craniocerebral injury

摘要 目的观察纳洛酮联合高压氧治疗颅脑损伤的临床效果。方法选取2018年12月-2020年12月武警特色医学中心收治的颅脑损伤患者172例,以随机数字表法分为试验组和对照组各86例。对照组单纯采用高压氧治疗,试验组采用纳洛酮联合高压氧治疗。比较2组患者的临床疗效、治疗前后神经功能、认知功能及痴呆情况。结果试验组患者治疗的总有效率为97.67%,显著高于对照组的87.21%(χ^(2)=6.740,P=0.009)。治疗后,2组患者的神经功能缺损程度评分量表(NIHSS)评分均显著低于治疗前,简易精神状态检查量表(MMSE)评分均显著高于治疗前,且试验组降低/升高幅度大于对照组(P<0.01)。试验组患者治疗后15 d、30 d及60 d的临床痴呆评定量表(CDR)评分均显著低于对照组(P<0.01)。结论纳洛酮联合高压氧治疗颅脑损伤可有效改善患者临床症状,优化患者的神经功能和认知功能,改善痴呆状态。 Objective To observe the clinical effect of naloxone combined with hyperbaric oxygen in the treatment of craniocerebral injury.Methods A total of 172 patients with craniocerebral injury admitted to the Armed Police Special Medical Center from December 2018 to December 2020 were selected,and they were divided into experimental group and control group by random number table method,86 cases in each group.The control group was treated with hyperbaric oxygen,and the experimental group was treated with naloxone combined with hyperbaric oxygen.The clinical efficacy,neurological function,cognitive function and dementia before and after treatment were compared between 2 groups.Results The total effective rate in experimental group was 97.67%,significantly higher than 87.21%in control group(χ^(2)=6.740,P=0.009).After treatment,the NIHSS score of 2 groups were significantly lower than before treatment,and the MMSE score were significantly higher than before treatment,the decrease/increase of experimental group was greater than that of control group(P<0.01).15,30 and 60 d after treatment,CDR score of experimental group were significantly lower than that of control group(P<0.01).Conclusion Naloxone combined with hyperbaric oxygen can effectively improve the clinical symptoms of patients with craniocerebral injury,optimize the neurological function and cognitive function of patients,and improve the dementia state.
