摘要
目的观察蠲痹历节清方辅助治疗膝关节痛风性关节炎湿热蕴结证的临床疗效。方法将64例膝关节痛风性关节炎湿热蕴结证患者采用随机数字表法分为治疗组和对照组,各32例。2组患者均行关节镜手术,对照组术后给予常规治疗,治疗组在对照组治疗方法的基础上给予蠲痹历节清方治疗。2组均以7 d为1个疗程,1个疗程后比较2组的临床疗效以及治疗前后的膝关节活动度、Lysholm膝关节功能评分、视觉模拟评分法(VAS)评分和血尿酸水平。结果对照组总有效率为87.50%(28/32),治疗组为90.62%(29/32),2组比较差异无统计学意义(P>0.05);治疗后2组膝关节活动度、Lysholm膝关节功能评分明显升高,VAS评分、血尿酸水平明显降低,与同组治疗前比较差异均有统计学意义(P<0.05),且治疗组Lysholm膝关节功能评分升高及VAS评分、血尿酸水平降低更明显,与对照组治疗后比较差异均有统计学意义(P<0.05)。结论蠲痹历节清方辅助治疗膝关节痛风性关节炎湿热蕴结证临床疗效确切,可有效改善患者的临床症状和膝关节功能,值得临床推广应用。
Objective To observe the clinical efficacy of Juanbi Lijieqing Fang(蠲痹历节清方)in adjuvant treatment of gouty arthritis of knee joint with damp-heat accumulation syndrome.Methods Sixty-four cases of gouty arthritis of knee joint with damp-heat accumulation syndrome were divided into treatment group and control group according to random number table method,with 32 cases in each group.The patients in both groups underwent arthroscopic surgery.The control group was given routine treatment after surgery,on this basis,the treatment group was given Juanbi Lijieqing Fang.Both groups took 7 days as 1 course of treatment.After 1 course of treatment,the clinical efficacy,range of motion(ROM)of knee joint,Lysholm knee scoring scale(LKSS)score,visual analogue scale(VAS)score and blood uric acid(BUA)level between the 2 groups before and after treatment were compared.Results The total effective rate of the control group was 87.50%(28/32),while that of the treatment group was 90.62%(29/32),and the difference was not statistically significant between the 2 groups(P>0.05).After treatment,ROM of knee joint and LKSS score significantly increased,while VAS score and BUA level significantly decreased in the 2 groups,and the difference was statistically significant than those in the same group before treatment(P<0.05),moreover,the increase of LKSS score as well as decrease of VAS score and BUA level in the treatment group was more significant,and the difference was statistically significant than that in the control group after treatment(P<0.05).Conclusion It has definite efficacy,can effectively alleviate the clinical symptoms and knee joint function to use Juanbi Lijieqing Fang(蠲痹历节清方)in adjuvant treatment of gouty arthritis of knee joint with damp-heat accumulation syndrome,which deserves clinical promotion and application.
