前牙美学区种植ASC螺丝固位修复的三维有限元分析

摘要 目的 通过有限元软件研究ASC角度螺丝通道螺丝固位修复种植牙在负载时各部位应力峰值及分布情况。方法 通过软件建立三组不同植入角度以及螺丝通道角度的模型。使用有限元软件对三组模型加载,对比模型各配件的应力峰值及分布情况。结果 使用25°螺丝通道的B组与直通道的A组比较,牙槽骨、适配器底座及牙冠应力峰值大,而修复螺丝和种植体的应力峰值略小于A组。结论 种植体植入角度唇倾比种植体植入角度理想者发生种植体边缘牙槽骨吸收和全瓷冠折裂的概率大。植入角度已经唇倾的种植体中,使用ASC角度螺丝通道螺丝固位修复,其负载时应力分布情况与非ASC固位组相似,但其拥有更好的美学修复效果。临床应用时,建议使用ASC角度螺丝通道螺丝固位修复已唇倾植入的种植体,以获得良好的美学修复效果。 Objective To investigate the stress distribution of screw fixation of implant repair with ASC(angle screw channel) using finite element software. Methods Three sets of models with different implanting angles and screw channel angles were established by software. Finite element software was used to load the models, comparing stress peak and stress distribution. Results Compared with group A, the peak stress of alveolar bone, adapter base and crown in group B with 25 °screw channel was higher, while the peak stress of restoration screw and implant was slightly lower. Conclusions The probability of alveolar bone resorption and all ceramic crown fracture was higher in group with lip inclination than in that with ideal implant angle. In the group of implant with lip inclination, the stress distribution of ASC angle screw fixation group was similar to that of non ASC fixation group, but it has better aesthetic effect.
