开腹与腹腔镜下手术治疗对胃肠肿瘤患者的营养状况及术后并发症的影响分析 被引量:3

Analysis of the Influence of Laparotomy and Laparoscopic Surgery on the Nutritional Status and Postoperative Complications of Patients with Gastrointestinal Tumors

摘要 目的分析对胃肠肿瘤患者行不同手术治疗对其营养状况以及术后并发症影响情况。方法随机选取2019年6月—2020年6月入该院进行治疗的90例胃肠肿瘤患者为研究对象。按照随机数表法分为对照组和观察组,每组45例。对照组接受开腹手术治疗,观察组接受腹腔镜下手术治疗。对比干预后两组患者手术情况、营养状况指标及术后并发症发生情况。结果治疗后,观察组术中出血量(46.37±3.25)mL、下床时间(1.38±0.33)d以及住院时间(10.22±1.50)d显著低于对照组术中出血量(120.43±5.45)mL、下床时间(2.09±0.43)d以及住院时间(15.87±2.98)d,差异有统计学意义(t=78.294、8.787、11.361,P<0.05);对照组手术耗时(185.88±2.98)min显著低于观察组手术耗时(236.01±4.23)min,差异有统计学意义(P<0.05);观察组总蛋白(60.22±4.01)g/L、白蛋白(194.02±10.23)mg/L、前白蛋白(36.71±3.87)g/L显著高于对照组总蛋白(56.02±3.43)g/L、白蛋白(170.87±9.98)mg/L、前白蛋白(29.98±3.34)g/L,差异有统计学意义(P<0.05);治疗后,观察组与对照组不良反应发生情况分别为2.22%、22.22%,差异有统计学意义(χ^(2)=8.389,P<0.05)。结论对胃肠肿瘤患者行腹腔镜下手术治疗效果更佳,能够有效提高术后营养状况指标值,并降低术后并发症发生率。 Objective To analyze the effects of different surgical treatments on patients with gastrointestinal tumors on their nutritional status and postoperative complications.Methods A total of 90 patients with gastrointestinal tumors admitted to the hospital from June 2019 to June 2020 were randomly selected as the research objects.According to random number table method,they were divided into control group and observation group,45 cases in each group.The control group received open surgery,and the observation group received laparoscopic surgery.Operation,nutritional status and postoperative complications were compared between the two groups after intervention.Results After treatment,the intraoperative blood loss(46.37±3.25)mL,bed time(1.38±0.33)d and hospitalization time(10.22±1.50)d of the observation group were significantly lower than those of the control group(120.43±5.45)mL,bed time(2.09±0.43)d,and hospitalization time(15.87±2.98)d,the difference was statistically significant(t=78.294,8.787,11.361,P<0.05).The operation time of the control group(185.88±2.98)min was significantly lower than that of the observation group(236.01±4.23)min,and the difference was statistically significant(P<0.05).Observation group total protein(60.22±4.01)g/L,albumin(194.02±10.23)mg/L,prealbumin(36.71±3.87)g/L were significantly higher than control group total protein(56.02±3.43)g/L,albumin(170.87±9.98)mg/L,prealbumin(29.98±3.34)g/L,the difference was statistically significant(P<0.05).After treatment,the incidence of adverse reactions in the observation group and the control group were 2.22%and 22.22%respectively,and the difference was statistically significant(χ^(2)=8.389,P<0.05).Conclusion Laparoscopic surgery is more effective for patients with gastrointestinal tumors,which can effectively improve the nutritional status index value after surgery and reduce the incidence of postoperative complications.
