品管圈在降低老年冠状动脉旁路移植术患者中重度营养不良发生率中的应用

摘要 目的 探讨品管圈在降低老年冠状动脉旁路移植术(CABG)患者中重度营养不良发生率中的应用效果。方法 选取宁夏某三级甲等医院心脏大血管外科2019年12月—2020年9月收治的96例老年CABG患者为研究对象,2019年12月—2020年4月品管圈开展前的47例患者为对照组,2020年5—9月品管圈开展后的49例患者为观察组,比较两组患者中重度营养不良发生率、营养相关指标、恶心/呕吐及疲乏症状改善情况和营养知识知晓情况。结果 观察组患者出院时中重度营养不良发生率低于对照组(P<0.05),观察组患者出院时血清总蛋白和血清白蛋白均高于对照组(P<0.05)。两组患者恶心症状改善情况比较差异无统计学意义(P>0.05),而观察组患者呕吐症状改善情况优于对照组,差异有统计学意义(P<0.05)。两组患者疲乏症状评分出现先升高后降低状况,时间比较差异有统计学意义(P_(时间)<0.05),交互比较差异无统计学意义(P_(交互)>0.05),组间比较差异无统计学意义(P_(组间)>0.05)。观察组患者营养相关知识各维度得分均高于对照组,差异有统计学意义(P<0.05)。结论 品管圈活动能有效降低老年CABG患者中重度营养不良发生率,改善患者术后相关营养指标及呕吐症状。 Objective To explore the application effect of quality control circle in reducing the incidence of moderate to severe malnutrition in elderly patients undergoing coronary artery bypass grafting(CABG).Methods To select 96 elderly patients with CABG who were admitted to the department of cardiovascular surgery in a tertiary hospital in Ningxia from December 2019 to September 2020 as the study objects.The 47 patients before the launch of the quality control circle from December 2019 to April 2020 were a control group,and the 49 patients after the launch of the quality control circle from May to September 2020 were an observation group to compare the incidence of moderate and severe malnutrition,nutrition-related indicators,improvement of nausea/vomiting and fatigue symptoms,and awareness of nutrition knowledge between the two groups.Results The incidence of moderate and severe malnutrition of the observation group were lower than those of the control group at the time of discharge(P<0.05).The serum total protein and serum albumin of the observation group were higher than those of the control group at the time of discharge(P<0.05).There was no signifi cant difference in the improvement of nausea symptoms between the two groups(P>0.05),but the improvement of vomiting symptoms in the observation group was better than that in the control group,and the difference was statistically significant(P<0.05).The fatigue symptom scores of the two groups of patients first increased and then decreased,and the difference in time was statistically significant(P<0.05),but there was no statistical significance in the interactive comparison(P>0.05),and there was no statistical difference in the comparison between the groups(P>0.05).The scores of each dimension of nutrition-related knowledge in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion The quality control circle activities can effectively reduce the incidence of moderate to severe malnutrition in elderly CABG patients,and improve postoperative related nutritional indicators and vomiting symptoms.
