摘要 目的探讨选择性头部亚低温疗法联合高压氧治疗新生儿缺氧缺血性脑病(HIE)的效果。方法选取2016年2月至2020年5月收治的80例HIE患儿进行研究,根据治疗方案不同将其分为对照组和观察组,各40例。对照组采用常规对症治疗及选择性头部亚低温疗法,观察组在对照组基础上加用高压氧。比较两组的治疗效果。结果观察组的治疗总有效率为97.50%,高于对照组的80.00%,差异具有统计学意义(P<0.05)。治疗后,两组的NSE、NGB、MBP水平均低于治疗前,且观察组低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的NBNA及Gesell评分均高于治疗前,且观察组高于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的IL-4、hs-CRP、TNF-α水平均低于治疗前,且观察组低于对照组,差异具有统计学意义(P<0.05)。结论选择性头部亚低温疗法联合高压氧治疗新生儿HIE的效果满意,可改善神经细胞因子及炎性因子水平,促进神经功能修复,值得推广。 Objective To explore the effect of selective head mild hypothermia therapy combined with hyperbaric oxygen in the treatment of neonatal hypoxic-ischemic encephalopathy(HIE).Methods A total of 80 neonates with HIE admitted from February 2016 to May 2020 for research.According to different treatment schemes,the neonates were divided into control group and observation group,with 40 cases in each group.The control group was treated with routine symptomatic treatment and selective head mild hypothermia therapy,and the observation group was treated with hyperbaric oxygen on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment of the observation group was 97.50%,which was higher than 80.00%of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of NSE,NGB and MBP in the two groups were lower than those before treatment,and those in the observation group were lower than the control group,the differences were statistically significant(P<0.05).After treatment,the scores of NBNA and Gesell in the two groups were higher than those before treatment,and those in the observation group were higher than the control group,the differences were statistically significant(P<0.05).After treatment,the IL-4,hs-CRP and TNF-αlevels in the two groups were lower than those before treatment,and those in the observation group were lower than the control group,the differences were statistically significant(P<0.05).Conclusion Selective head mild hypothermia therapy combined with hyperbaric oxygen in the treatment of neonatal HIE has a satisfactory effect,it can improve the levels of neurocytokines and inflammatory factors,and promote the repair of nerve function,which is worthy of promotion.
