听觉口语法对听障儿童语言康复训练的效果 被引量:4

The Effects of Auditory Verbal Therapy on Individualized Speech Rehabilitation Training of Hearing-Impaired Children

摘要 目的探讨听觉口语法对听障儿童语言康复训练的效果。方法选取我院2018年8月~2020年8月收治的32例听障儿童,采用随机数表法分为观察组与对照组各16例,对照组进行视听训练,观察组结合听觉口语法进行个性化语言康复训练。评估干预前后两组患儿听觉能力、语言功能、平均语言年龄、言语可懂度,进行单因素及多因素回归分析,筛选出影响患儿语言功能水平的因素。结果干预6个月后,两组患儿听觉能力各指标及总分均有显著提升(P<0.05);组间对比,除单音节声调识别、单音节词识别无显著差异外,其余各得分及总分观察组均显著优于对照组(P<0.05)。干预后,两组患儿平均语言年龄、语言功能、言语可懂度均有所改善,观察组显著高于对照组(P<0.05)。logistic回归分析结果显示,患儿术前智力水平、学习能力、康复方式、康复时间、人工耳蜗植入年龄、术后康复方式为听障儿童干预后语言功能的独立影响因素(P<0.05),ROC曲线下面积为0.898。结论听觉口语法可明显改善听障儿童的听觉、言语、语言、认知及沟通能力,应尽早进行系统干预,发挥家庭教育功能。 Objective To explore the effect of auditory verbal therapy on the individualized language rehabilitation training of hearing-impaired children.Methods 32 cases of hearing-impaired children admitted to our hospital from August 2018 to August 2020 were randomly selected and divided into can observation group(n=16)and a control group(n=16).The control group received traditional audio-visual combination training intervention,while the observation group received individualized speech rehabilitation training by combining auditory verbal therapy.Assessment test methods and observation methods were used to evaluate the grades of auditory ability,average language age,language function and speech intelligible degree of children in the two groups before and after intervention.A single factor analysis and factor regression analysis were used to screen out the factors affecting the level of language function of children in the two groups before and after intervention.Results After 6 months of treatment,all indexes and total scores of auditory ability assessment were increased in the two groups(P<0.05).The comparison between groups showed that the scores of monosyllabic tone recognition and monosyllabic word recognition were not significantly different,and the scores of other items and total scores in the observation group were significantly better than those in the control group(P<0.05).After treatment,the mean language age,language function and speech intelligability grade of children in both groups were improved,and the increase rate in observation group was significantly higher than that in control group(P<0.05).Logistic regression analysis showed that preoperative intelligence level,preoperative learning ability,preoperative rehabilitation mode,preoperative rehabilitation time,age of cochlear implantation,and postoperative rehabilitation mode were independent influencing factors for the improvement of language function of hearing-impaired children after treatment(P<0.05),and the area under the ROC curve(AUC)was 0.898.Conclusion Auditory verbal therapy can significantly improve the hearing ability,speech,language,cognitive and communication rehabilitation effect of children with hearing impairment.Clinically,intervention should be carried out as early as possible and as long as possible to give play to the function of family education.
