摘要
目的探讨调气化痰汤预防缺血性中风病复发患者的临床疗效,为预防缺血性中风病复方提供参考依据。方法采用前瞻、随机、对照的临床试验设计方法,将丰润区中医医院在2018年3月至2019年6月收治的100例符合初发缺血性中风病患者作为研究对象,采取随机数字表法将其分成治疗组与对照组,每组50例,2组均给予脑血管病的常规治疗,治疗组加用中药调气化痰汤1剂/d口服,疗程为30 d,在治疗前、治疗后按照《中风病中医诊断疗效评定标准》、美国国立卫生研究院卒中量表(NIHSS)评定其临床疗效,观察随访180 d内2组患者卒中事件发生情况。比较2组治疗前后疗效、神经功能[美国国立卫生研究院卒中量表(NIHSS)]、复发率、血脂变化[总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、三酰甘油(TG)]、安全性。结果2组治疗后疗效比较,差异有统计学意义(P<0.05)。2组患者NIHSS评分在治疗后与基线比较,差异均有统计学意义(P<0.05),治疗前2组差异均无统计学意义(P>0.05),治疗后2组评分差异有统计学意义(P<0.05),治疗组NIHSS评分明显更低(P<0.05)。随访180 d内,治疗组复发率低于对照组,差异有统计学意义(P<0.05)。2组均能降低缺血性中风患者血清中的LDL-C、TG、TC水平,但治疗组降低幅度更大,组间比较差异有统计学意义(P<0.05)。2组患者肝肾功能各项指标均无异常,患者也未出现明显不良反应。结论应用调气化痰汤对出发缺血性中风病患者进行治疗能取得很好的临床疗效,能显著患者神经功能,调节患者血脂水平,预防疾病复发,且用药安全性较高,此治疗方案具有积极的临床应用价值。
Objective To investigate the clinical effects of Tiaoqihuatan decoction in preventing the recurrence of ischemic stroke.Methods The prospective,randomized,and controlled clinical trial design methods were used,and 100 patients with primary ischemic stroke who were treated in our hospital from March 2018 to June 2019 were enrolled in the study,who were randomly divided into control group and observation group,with 50 cases in each group.The patients in both groups were treated by conventional therapy,on this basis,the patients in observation group were treated by Tiaoqihuatan decoction one dose a day,for 30 days.The curative effects were evaluated according to the Standards for Evaluation of TCM Diagnosis and Efficacy of Stroke and the National Institutes of Health Stroke Scale(NIHSS).After 180-day follow up,the incidence of stroke events,the clinical efficacy,and nerve function of patients,recurrence rate,blood lipid change and safety were observed and compared between the two groups.Results There were significant differences in the curative effects and NIHSS scores between the two groups(P<0.05),moreover,the NIHSS scores in observation group were significantly lower than those in control group(P<0.05).After 180-day follow up,the relapse rate in observation group was significantly lower than that in control group(P<0.05).In addition the serum levels of LDL-C,TG,TC in both groups were decreased,moreover,which in observation group were significantly lower than those in control group(P<0.05).Furthermore no abnormal live and kidney indxes as well as adverse reactions were observed in both groups.Conclusion Tiaoqihuatan decoction has obvious curative effects on ischemic stroke,which can significantly improve the nerve function,regulate the blood lipid levels of patients,and prevent disease recurrence,with better safety,therefore,which has important clinical application value.
