摘要
目的评价轻度尿道下裂Duckett术后延长导尿管留置时间对术后并发症(尿瘘及尿道狭窄)发生率的影响。方法收集2015年1月至2021年1月郑州大学第一附属医院首次行一期尿道成形术(横裁包皮岛状皮瓣管状尿道成形术即Duckett术式,以下简称Duckett术式)治疗轻度尿道下裂患儿的临床资料。按术后导尿管留置时间不同分为两组,对照组51例患儿术后7~10 d拔出导尿管,平均(8.51±1.07)d;试验组45例患儿术后14~30 d拔出导尿管,平均(17.20±3.14)d。比较两组患儿术后并发症(尿瘘及尿道狭窄)发生率的情况。结果本次回顾性研究分析中,对照组患儿尿瘘发生9例,发生率为17.65%;尿道狭窄发生8例,发生率为15.69%;术后总并发症发生率为33.34%。试验组患儿尿瘘发生1例,发生率为2.22%;尿道狭窄发生1例,发生率为2.22%;术后总并发症发生率为4.44%,两组比较,差异有统计学意义(P<0.05)。结论轻度尿道下裂Duckett术后延长导尿管留置时间至14 d后[平均(17.20±3.14)d],对术后尿瘘及尿道狭窄发生率的降低有影响,可明显降低轻度尿道下裂术后总的并发症发生率,较大的提高手术成功率。
Objective To evaluate the effect of prolonging the urinary catheter indwelling time on the incidence of postoperative complications(urinary fistula and urethral stricture)after Duckett operation for mild hypospadias.Methods The clinical data of children with mild hypospadias who received first-stage urethroplasty(the operation was the cross-cut foreskin island flap tubular urethroplasty,namely the Duckett operation,hereinafter referred to as the Duckett operation)from January 2015 to January 2021 of the First Affiliated Hospital of Zhengzhou University were collected.According to the postoperative urinary catheter indwelling time,they were divided into the control group with 51 children who had their catheters removed 7-10 days after operation,with an average of(8.51±1.07)days,and the experimental group with 45 children who had their catheters removed 14-30 days after operation,with an average of(17.20±3.14)days.The incidence of postoperative complications(urinary fistula and urethral stricture)was compared between the two groups.Results In the control group,9 children had urinary fistula with an incidence rate of 17.65%.8 children had urethral stricture with an incidence rate of 15.69%;the total postoperative complication rate was 33.34%.In the experimental group,1 child had urinary fistula with an incidence rate of 2.22%;1 child had urethral stricture with an incidence rate of 2.22%;and the total postoperative complication rate was 4.44%.There were statistically significant differences between the two groups(P<0.05).Conclusion Prolonging the urinary catheter indwelling time to 14 days(average time[17.20±3.14]d)can reduce the incidence of postoperative urinary fistula and urethral stricture,and significantly decrease the overall incidence of complications after Duckett operation for mild hypospadias,which greatly improves the success rate of surgery.
