摘要
窦材传承《内经》和道家思想,认为阳气为人身之主宰,饮食生冷、情志内伤或妄用寒凉转下法等皆易损伤阳气,因此临证以保扶阳气为本。基于“脾为五脏之母,肾为一身之根”的学术观点,窦材常以脾肾之阳判断人身阳气的盛衰,治疗疾病注重补益脾肾之阳。《扁鹊心书》上卷专篇论述“五等虚实”内容,以脾阳、肾阳的虚损程度作为切入点诊察病情病势,提出“平气、微虚、甚虚、将脱、已脱”5种阳气虚损程度,以保扶阳气为治疗原则,灸药并用固护脾肾之本为治疗关键。通过医案剖析5种治疗方法的具体应用,传承窦材“辨证施治,药证相对,缓急得当”的治疗思路,为后世重视扶阳、补益脾肾、灸药并用的临床应用提供借鉴和参考。
Dou Cai inherited the philosophy of Nei Jing and Taoism and believes that Yang Qi is the master of the human body,and that Yang Qi is easily damaged by eating cold food,emotional and mental trauma,or by using cold and cold methods.Therefore,protecting and supporting yang qi is fundamental in clinical practice.Based on the academic view that"the spleen is the mother of the five viscera and the kidney is the root of the whole body",Dou Cai often uses the yang of the spleen and kidney to judge the strength and weakness of the yang of the human body and focuses on nourishing the yang of the spleen and kidney in the treatment of diseases.In VolumeⅠof the Bian Que Xin Shu,a special section is devoted to the"Five Levels of Deficiency",in which the degree of deficiency of the spleen and kidney yang is used as an entry point to diagnose the condition,and five levels of yang deficiency are proposed,including"flat qi,slightly deficient,very deficient,about to be removed,and already removed".Protecting and supporting Yang Qi as treatment principle,the key to treatment is to protect the spleen and kidney by using moxibustion and medicine.This paper attempts to analyse the specific application of the five treatment methods through medical cases,and to pass on the therapeutic ideas of Dou Cai of discriminating treatment,relative agreement between medicine and evidence,and appropriate priority and urgency,so as to provide a reference for future generations to emphasize the clinical application of supporting yang,tonifying the spleen and kidney,and combined use of moxibustion and drug.
