摘要
目的探讨经掌侧入路锁定加压钢板固定治疗老年桡骨远端骨折的疗效。方法选择2018年4月-2019年10月收治的老年桡骨远端骨折患者57例临床资料,根据治疗方式的不同将其分为保守组(30例)和手术组(27例)。保守组进行手法复位石膏外固定治疗,手术组进行经掌侧入路锁定加压钢板固定术治疗。比较分析两组的Cooney腕关节评分、X线检查指标、并发症情况。结果手术组Cooney腕关节评分的功能、活动范围、握力、总分均高于保守组(P<0.05);治疗后手术组的掌倾角、尺偏角、桡骨缩短均优于保守组(P<0.05);保守组的并发症总发生率(16.66%)与手术组的并发症总发生率(7.40%)比较差异无统计学意义(P>0.05)。结论经掌侧入路锁定加压钢板固定治疗老年桡骨远端骨折能够提高腕关节功能,促进骨折的愈合。
Objective To investigate the efficacy of volar approach locking compression plate fixation in the treatment of elderly distal radius fractures.Methods Fifty-seven cases of elderly patients with distal radius fracture in our hospital from April 2018 to October 2019 were selected and divided into conservative group(30 cases)and operation group(27 cases)according to different treatment methods.The conservative group was treated with manipulative reduction and plaster external fixation,and the operation group was treated with volar approach locking compression plate fixation.The Cooney wrist score,X-ray examination index and complications of two groups were compared.Results The function,range of motion,grip strength and total score of Cooney wrist score in the operation group were higher than those in the conservative group(P<0.05).After treatment,the palmar inclination,ulnar deviation and radial shortening in the operation group were better than those in the conservative group(P<0.05).There was no significant difference in the total incidence of complications between the conservative group(16.66%)and the operation group(7.40%)(P>0.05).Conclusion Volar approach locking compression plate fixation in the treatment of elderly distal radius fractures can improve wrist function and promote fracture healing.
