2020年浙江省衢州市新诊断HIV/AIDS病例HIV-1基因型及耐药分析

Analysis of HIV-1 genotype and drug resistance in newly diagnosed HIV/AIDS cases in 2020 in Quzhou City,Zhejiang Province

摘要 目的探讨衢州市2020年新报告HIV/AIDS病例HIV-1毒株基因亚型分布和原发性耐药情况。方法收集2020年衢州市新诊断且未经治疗的HIV/AIDS样本,采用In-house法扩增pol基因并测序,构建系统进化树分析基因亚型。通过CPR6.0在线软件分析耐药情况。结果从新报告的63例HIV/AIDS样本中,成功获取50例合格序列,主要亚型为CRF01_AE(42%,21/50)和CRF07_BC(26%,13/50),其余亚型包括CRF08_BC(6例),B亚型(6例),CRF55_01B(4例)。原发性耐药率占12.0%(6/50),NNRTI类耐药发生率为8.0%(4/50),NRTI类耐药发生率为4.0%(2/50),PI类耐药发生率为2.0%(1/50)。与耐药相关的亚型有CRF55_01B、B亚型和CRF01_AE,CRF55_01B亚型原发性耐药率最高,达75.0%(3/4),且位点均在NNRTI的M46L,B’和CRF01_AE亚型原发性耐药率分别为16.7%(1/6)和9.5%(2/21)。结论衢州市新报告HIV/AIDS病例HIV-1遗传基因多样化,原发性耐药率处于中度水平。 Objective To investigate the subtype distribution and the primary drug-resistant mutation among newly diagnosed HIV-AIDS individuals in 2020 in Quzhou City.Methods Newly diagnosed and untreated HIV/AIDS samples from Quzhou City In 2020 were collected.pol gene was amplified and sequenced by in-house method,and phylogenetic tree was constructed to analyze gene subtypes.Drug resistance was analyzed by CPR6.0 online software.Results Qualified sequences were successfully obtained from 63 newly reported HIV/AIDS samples in 50 cases,the main subtypes were CRF01_AE(42%,21/50)and CRF07_BC(26%,13/50),and the other subtypes included CRF08_BC(6 cases)and subtype B(6 cases).CRF55_01 B(4 cases).Primary drug resistance rate was 12.0%(6/50),drug resistance rate of NNRTI type was 8.0%(4/50),drug resistance rate of NRTI type was 4.0%(2/50),and drug resistance rate of PI type was 2.0%(1/50).The subtypes associated with drug resistance were CRF55_01 B,B subtype and CRF01_AE.The primary drug resistance rate of CRF55_01 B subtype was the highest,reaching 75.0%(3/4),and all the sites were located in M46 L of NNRTI.The primary drug resistance rates of B’and CRF01_AE subtypes were 16.7%(1/6)and 9.5%(2/21),respectively.Conclusion HIV-1 subtype epidemic strains of newly diagnosed HIV-infected individuals were diverse in Quzhou City,and the rates of HIV-1 primary drug-resistant mutations in naive patients is moderate.
