“3C”疗法对糖尿病合并糖尿病周围神经病变患者病情控制的影响

Effect of “3C” Therapy on Disease Control of Patients with Diabetes Mellitus and Diabetic Peripheral Neuropathy

摘要 目的:探究“3C”疗法对糖尿病合并糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)患者病情控制的影响。方法:选择2019年3月-2021年3月在本溪市中心医院诊治的116例糖尿病合并DPN患者,按随机数字表法将患者分为对照组57例与观察组59例。对照组接受胰岛素泵持续皮下输注(continuous subcutaneous insulin infusions,CSII)治疗,观察组给予“3C”疗法,即实时动态血糖监测(continuous glucose monitoring system,CGMS)、CSII、糖尿病管理软件(Carelink)联合疗法。治疗2周后,比较两组疗效、空腹血糖(fasting plasma glucose,FPG)、糖化血红蛋白(glycosylated hemoglobin,HbA1c)、多伦多临床评分系统(Toronto clinical scoring system,TCSS)评分、生活质量及正中神经与腓神经的运动、感觉神经传导速度。结果:观察组总有效率为88.14%,高于对照组的71.93%(P<0.05)。治疗后,两组FPG、HbA1c、TCSS评分均较治疗前明显降低,且观察组FPG、HbA1c、TCSS评分均低于对照组(P<0.05)。治疗后,两组运动神经传导速度及感觉神经传导速度均较治疗前明显升高,且观察组运动神经传导速度及感觉神经传导速度均高于对照组(P<0.05)。治疗后,观察组躯体疼痛、精力、总体健康、生理功能评分均高于对照组(P<0.05)。结论:“3C”疗法能够明显缓解糖尿病合并糖尿病周围神经病变患者病情,有效控制血糖,改善神经传导功能与生活质量。 Objective:To explore the effect of “3C” therapy on disease control of patients with diabetes mellitus and diabetic peripheral neuropathy (DPN).Method:A total of 116 patients with diabetes mellitus complicated with DPN from March 2019 to March 2021 in Benxi Central Hospital were selected and divided into control group (n=57) and observation group (n=59) according to random number table method.The control group received continuous subcutaneous insulin infusions (CSII),and the observation group received “3C” therapy,continuous glucose Monitoring System (CGMS),CSII,and Diabetes management software (Carelink) combined therapy.2 weeks after treatment,efficacy,fasting plasma glucose (FPG),glycosylated hemoglobin (HbA1c),Toronto clinical scoring system (TCSS) score,quality of life,motor and sensory nerve conduction velocities of median and peroneal nerves were compared between the two groups.Result:The total effective rate of observation group was 88.14%,which was higher than 71.93% of control group (P<0.05).After treatment,the FPG,HbA1c and scores of TCSS in two groups were significantly lower than those before treatment,and the FPG,HbA1c and scores of TCSS in observation group were lower than those in control group (P<0.05).After treatment,the motor nerve conduction velocity and sensory nerve conduction velocity in both groups were significantly higher than those before treatment,and the motor nerve conduction velocity and sensory nerve conduction velocity in the observation group were higher than those in the control group (P<0.05).After treatment,the scores of body pain,energy,general health and physiological function in observation group were higher than those in control group (P<0.05).Conclusion:“3C” therapy can significantly relieve the condition of patients with diabetes mellitus and DPN,effectively control blood glucose,and improve nerve conduction and quality of life.
